目的评价农村糖尿病患者生活质量。方法调查416例医院门诊的农村糖尿病患者个人基本资料和疾病状况,用中文版EQ-5D生活质量量表评价患者的生活质量,采用Logistic回归和多元线性回归方法分析影响生活质量的相关因素。结果农村糖尿病患者在5个维度中,存在中、重度困难比例分别为“行动能力”21.15%,“自己照顾自己能力”11.54%,“日常活动能力”17.31%,“疼痛/不适”38.46%和“焦虑/抑郁”47.12%。EQ-5D平均效用值0.835±0.197,EQ-VAS评分均值(61.48±16.47)分。Logistic回归分析显示,并发症是5个维度共同的影响因素,年龄是“运动能力”、“日常活动能力”、“疼痛/不适”和“焦虑/抑郁”等4个维度共同的影响因素,性别和Hb A1C是“疼痛/不适”的两个影响因素。多元线性回归分析显示,并发症数量对EQ-5D效用值有明显影响,并发症数量和Hb A1C对EQ-VAS评分也有明显影响。结论农村糖尿病患者生活质量明显低于一般人群,焦虑/抑郁是最主要问题,并发症是患者生活质量下降的主要影响因素。
Objective To evaluate the quality of life of patients with diabetes in rural areas. Methods A total of 416 hospitalized patients with diabetes in rural areas were investigated for their personal basic information and disease status. The quality of life was evaluated using the Chinese version of the EQ-5D Quality of Life Scale. Logistic regression and multivariate linear regression were used to analyze the factors influencing quality of life. Results Among the five dimensions of rural diabetics, there were 21.15% of the moderate and severe difficulties in “moderate ability”, “11.54% of self-care ability”, “17.31% of daily activity ability”, “ Pain / discomfort ”38.46% and “ anxiety / depression ”47.12%. EQ-5D average utility value 0.835 ± 0.197, EQ-VAS score average (61.48 ± 16.47) points. Logistic regression analysis showed that complications were the common influencing factors in 5 dimensions, and age was “exercise ability ”, “daily activity ability ”, “pain / discomfort ” and “anxiety / depression ” 4 Factors common to the dimensions, gender, and Hb A1C are the two “pain / discomfort” factors. Multivariate linear regression analysis showed that the number of complications had a significant effect on the EQ-5D utility value, and the number of complications and Hb A1C also had a significant effect on EQ-VAS scores. Conclusion The quality of life of patients with diabetes in rural areas is significantly lower than that of the general population. Anxiety / depression are the most important problems. Complications are the main factors affecting the quality of life of patients with diabetes.
