目的探讨经皮明胶海绵栓塞冠状动脉法建立猪心肌梗死模型的方法,观察猪急性心肌梗死(AMI)早期血清缺血修饰白蛋白(IMA)的动态变化。方法选用中华小型猪16只,麻醉后经股动脉穿刺,行冠脉造影后经微导管注入明胶海绵堵闭猪钝缘支(OM)支,分别在术前及术后10 min、30 min、1、2、3、4、6、8、12、24、48 h抽取猪耳缘静脉血观察IMA的动态演变,并于1、3、5、7、10 d后处死取出心脏,取目标心肌做病理学检查。结果 15头猪心肌梗死造模成功,心电图、心脏标本、病理切片均显示典型心肌梗死改变。IMA在猪心肌梗死后10 min后即开始升高,6 h达峰值,48 h降至正常范围。结论经皮冠状动脉明胶海绵栓塞法建立猪AMI模型科学可靠,简单易行,可重复性强。血清IMA的水平的动态检测对于AMI的早期诊断有一定的临床意义。
Objective To investigate the method of percutaneous gelatin sponge embolization coronary artery method to establish pig model of myocardial infarction and to observe the dynamic change of early ischemic albumin (IMA) in pigs with acute myocardial infarction (AMI). Methods Sixteen Chinese miniature pigs were selected. After anesthesia, the femoral artery puncture and coronary angiography were performed. After the microcirculation was implanted with gelatin sponge, the OM obstructions were closed. The animals were sacrificed at 10 min, 30 min, At 1, 2, 3, 4, 6, 8, 12, 24 and 48 h, the dynamic changes of IMA were observed and the hearts were removed after 1,3,5,7 and 10 days. Do pathological examination. Results 15 pigs successfully established myocardial infarction, ECG, heart specimens, pathological sections showed a typical myocardial infarction changes. IMA began to increase 10 min after myocardial infarction in pigs, peaked at 6 h, and dropped to the normal range at 48 h. Conclusion The establishment of porcine AMI model by percutaneous coronary gelatin sponge embolization is scientific and reliable, simple and reproducible. Dynamic detection of serum IMA level for the early diagnosis of AMI has some clinical significance.
