目的结合转移体内迟发超敏反应检测法初步探讨外周血中CD4+CD25+FoxP3+Treg的比例对亲体肾移植受者免疫状态评估的作用。方法收集西南医院肾科门诊随访的2005-2008年间手术的亲体肾移植供者和受者各30例,进行转移体内迟发超敏反应检测(trans-vivodelayed type hypersensitivity-assay,Transvivo-DTH)实验,根据实验结果将患者分为耐受组和排斥组;运用流式细胞技术检测2组中受者外周血单个核细胞(PBMCs)中CD4+CD25+FoxP3+Treg比例。结果耐受组和排斥组裸鼠掌肿胀(同时注入受者PBMCs和供者抗原的脚掌)厚度分别为(1.72±0.44)×10-2、(8.05±2.26)×10-2mm,前者明显高于后者(P<0.05),2组受者外周血中CD4+CD25+FoxP3+Treg的比例分别为(9.23±1.79)%和(1.03±1.04)%,耐受组明显高于排斥组(P<0.05)。耐受组和排斥组裸鼠脚掌皮下炎性水肿的厚度均与外周血中CD4+CD25+FoxP3+Treg的比例呈负相关(P<0.05,r分别为0.82和0.87)。结论受者外周血中CD4+CD25+FoxP3+Treg的比例能够反应受者T细胞对供者抗原发生免疫反应的程度。外周血中CD4+CD25+FoxP3+Treg的比例在发现移植受者是否已有耐受方面有一定的参考价值。
OBJECTIVE: To investigate the role of the ratio of CD4 + CD25 + FoxP3 + Treg in the peripheral blood in the assessment of immune status of recipients with renal transplant recipients in combination with metastasis delayed-type hypersensitivity assay. Methods Thirty cases of donors and recipients of renal transplantation from 2005 to 2008 in the Department of Nephrology of Southwest Hospital were enrolled in this study. Trans-vivodelayed type hypersensitivity-assay (Transvivo-DTH) According to the experimental results, the patients were divided into tolerated group and rejection group. The proportion of CD4 + CD25 + FoxP3 + Treg in peripheral blood mononuclear cells (PBMCs) was detected by flow cytometry. Results The thickness of the palpitations of the nude mice in the tolerated and rejected groups (1.72 ± 0.44 × 10-2, (8.05 ± 2.26) × 10-2mm, respectively) was significantly higher In the latter group (P <0.05), the percentage of CD4 + CD25 + FoxP3 + Tregs in peripheral blood of the two groups were (9.23 ± 1.79)% and (1.03 ± 1.04)%, respectively, P <0.05). The thickness of subcutaneous inflammatory edema in the nude mice of the tolerated group and the rejection group was negatively correlated with the ratio of CD4 + CD25 + FoxP3 + Treg in peripheral blood (P <0.05, r = 0.82 and 0.87, respectively). Conclusion The proportion of CD4 + CD25 + FoxP3 + Treg in peripheral blood of recipients can reflect the degree of T cell immune response to donor antigen. The proportion of CD4 + CD25 + FoxP3 + Treg in peripheral blood has a certain reference value in discovering whether transplant recipients have tolerance.
