目的:探讨聚合型髌骨环抱器治疗髌骨骨折的临床效果。方法:选取2016年6月至2019年8月昆明医科大学附属延安医院呈贡区医院(呈贡区人民医院)骨科收治的39例髌骨骨折患者,根据手术方法不同将患者分为环抱器组(n n=20)与张力带组(n n=19),男25例,女14例,年龄(41±22)岁,年龄范围为19~63岁。比较两组患者术后4、8、12周膝关节活动范围、临床愈合时间、与健侧对比屈曲功能缺失情况。n 结果:环抱器组患者术后4、8、12周膝关节屈曲角度(90.0°±13.2°、110.0°±8.3°、120.0°±9.0°)活动范围优于张力带组(74.0°±10.5°、89.0°±10.5°、105.0°±10.5°);临床愈合时间[(11.0±3.3)周]短于张力带组[(12.0±4.5)周],差异均有统计学意义(n P<0.001)。两组患者术后24周患侧膝关节对比健侧伸直活动正常,环抱器组屈曲活动缺失(5.0°±3.3°)优于张力带组(12.0°±6.7°),差异有统计学意义(n P<0.001)。n 结论:聚合型髌骨环抱器治疗髌骨骨折,患者髌骨周围软组织刺激症状较少,内固定更为稳定,膝关节早期功能康复效果较好,临床愈合时间相对缩短,治疗效果好。“,”Objective:To investigate the clinical effect of the treatment of patella fracture with the polymeric patella ring device.Methods:A retrospective study was performed on 39 cases of patients with patella fracture who were admitted from June 2016 to August 2019.According to the different surgical methods, patients were divided into the polymeric patella ring device group(n n=20)and tension band group(n n=19), 25 males and 14 females, aged(41±22)years, ranging from 19 to 63 years.The range of motion of knee joint, clinical healing time and the loss of flexion function were compared between the two groups at 4, 8 and 12 weeks after surgery.n Results:The range of motion of knee flexion angles[(90.0°±13.2°), (110.0°±8.3°)and(120.0°±9.0°)]at 4, 8 and 12 weeks after surgery of polymeric patella ring device group was better than that of the tension band group[(74.0°±10.5°), (89.0°±10.5°)and(105.0°±10.5°)]. Clinical healing time[(11.0±3.3)weeks]of the polymeric patella ring device group was shorter than that of the tension band group[(12.0±4.5)weeks], with statistically significant differences(n P<0.001). The flexion activity of the affected side of the knee joint was normal in the two groups 24 weeks after the operation, while the loss of flexion activity in the polymeric patella ring device group(5.0°±3.3°)was better than that in the tension band group(12.0°±6.7°), and the difference was statistically significant(n P<0.001).n Conclusion:In the treatment of patella fracture, the patients had less symptoms of stimulation of the surrounding soft tissues of the patella, more stable internal fixation, better early functional rehabilitation effect of the knee joint, relatively shorter clinical healing time, and better treatment effect.
