目的 研究 2型糖尿病 (T2DM)患者尿白蛋白排泄 (UAE)的主要影响因素。 方法 观察T2DM并糖尿病肾病组 (DN组 )和非DN组 (NDN组 )各 79例与健康对照组 5 0名之间胰岛素样生长因子 1(IGF 1)、空腹胰岛素 (FINS)、空腹血糖 (FBG)、胰岛素敏感指数 (ISI)、糖化血红蛋白 (HbA1 c)、总胆固醇 (TC)、甘油三酯 (TG)、高密度脂蛋白 胆固醇 (HDL c)、低密度脂蛋白 胆固醇 (LDL c)、收缩压(SBP)、舒张压 (DBP)、体重指数 (BMI)等因素的变异 ,并分析其与UAE的关系。 结果 ( 1) 3组间性别、BMI差异均无显著意义 ( χ2 =0 .35 8,F =1.5 3,均P >0 .0 5 ) ,DN组和NDN组间年龄、FBG、HbA1 c差异均无显著意义 (均P >0 .0 5 ) ;( 2 )与NDN组和对照组比较 ,DN组SBP、DBP、IGF 1均显著升高 (F为33 .0 8、8.47和 9.15 ,均P <0 .0 1) ,TG显著升高、HDL C明显降低 (F为 4.37和 4.0 0 ,均P <0 .0 5 ) ;( 3)DN组FINS显著高于NDN组、ISI显著低于NDN组 ,两组FINS均显著高于健康对照组、ISI均显著低于健康对照组 (F为 2 9.41和 37.77,均P <0 .0 1)。 ( 4)以UAE为应变量 ,BMI、FINS、ISI、FBG、HbA1 c、TC、TG、HDL c、LDL c为自变量 ,α为 0 .0 5 ,多元回归显示FINS、FBG和TG与UAE呈正相关 (R2 =0 .171) ;自变量加入IGF 1后 ,I
Objective To study the main influencing factors of urinary albumin excretion (UAE) in type 2 diabetes mellitus (T2DM). Methods The levels of insulin-like growth factor 1 (IGF 1), fasting insulin (FINS), fasting plasma glucose (FBG) and serum fasting insulin (FBP) were measured in 79 diabetic patients with and without DN group FBG, ISI, HbA1 c, TC, TG, HDL c, LDL c, Systolic blood pressure (SBP), diastolic blood pressure (DBP), body mass index (BMI) and other factors, and analyzed its relationship with UAE. Results There was no significant difference in gender and BMI between the three groups (χ2 = 0.358, F = 1.53, all P> 0.05). The difference of age, FBG and HbA1c between DN group and NDN group (2) Compared with NDN group and control group, the SBP, DBP and IGF 1 in DN group were significantly increased (F = 33.08, 8.47 and 9.15, both P <0.01), TG significantly increased, HDL C significantly decreased (F 4.37 and 4.0 0, both P0.05); (3) FINS of DN group was significantly higher than that of NDN group, ISI was significantly lower than NDN group, FINS in both groups were significantly higher than the healthy control group, ISI were significantly lower than the healthy control group (F = 9.441 and 37.77, P <0.01). (4) UAE was used as a dependent variable, and BMI, FINS, ISI, FBG, HbA1c, TC, TG, HDLc and LDLc were independent variables and α was0.0.5. Multiple regression showed that FINS, FBG and TG were correlated with UAE (R2 = 0.171). After the independent variable was added to IGF1,
