目的 :了解促炎、抗炎细胞因子在急性感染患者中的动态变化 ,并探讨中药 912液的治疗作用。方法 :采用鲎试剂基质偶氮显色定量法及酶联免疫吸附法 ,对 4 0例急性感染患者 (随机分为两组 ,普通治疗组 2 0例 ,中药 912液加普通治疗组 2 0例 )测定治疗前后血浆内毒素、血清促炎因子 (TNFα、IL 6)、抗炎因子 (sTNF RI、IL 4、IL 10 )的水平。结果 :急性感染患者早期即出现内毒素血症 ,促炎因子 (TNFα、IL 6)及抗炎因子 (sTNF RI、IL 4、IL 10 )均呈现不同程度的升高 ,两者水平变化基本平行 ,尤以IL 6、sTNF RI、IL 4、IL 10明显 ;TNFα、IL 6、sTNF RI与APACHEⅢ评分呈明显正相关。经治疗随病情好转 ,内毒素及炎症介质呈现不同程度的下降 ,而中药 912液加普通治疗组内毒素及TNFα、IL 6、IL 10下降幅度显著 ,优于普通治疗组 (P <0 0 5)。结论 :内毒素、促炎因子及抗炎因子在急性感染中起重要作用 ,可作为评估感染性疾病病情严重程度的指标 ,中药 912液使内毒素、TNFα、IL 6、IL 10水平下降 ,抑制sTNF RI下降 ,促进疾病恢复
Objective: To understand the dynamic changes of proinflammatory and anti-inflammatory cytokines in patients with acute infection, and to explore the therapeutic effect of Chinese medicine 912 solution. Methods: Forty cases of acute infection were randomly divided into two groups (20 cases in the ordinary treatment group, 20 cases in the traditional Chinese medicine 912 solution and the ordinary treatment group) with the method of colorimetric determination of azo reagent and enzyme-linked immunosorbent assay ) Were measured before and after treatment of plasma endotoxin, serum proinflammatory cytokines (TNFα, IL 6), anti-inflammatory cytokines (sTNF RI, IL 4, IL 10) levels. Results: In the early stage of acute infection, endotoxemia appeared, and the levels of proinflammatory cytokines (TNFα, IL 6) and anti-inflammatory cytokines (sTNF RI, IL 4 and IL 10) increased to some extent, , Especially IL 6, sTNF RI, IL 4 and IL 10, while TNFα, IL 6 and sTNF RI were positively correlated with APACHE Ⅲ scores. After treatment, the endotoxin and inflammatory mediators decreased in varying degrees. However, the decrease of endotoxin, TNFα, IL 6 and IL 10 in the 912 Chinese medicine plus normal treatment group was significantly better than that in the normal treatment group (P <0.05) ). Conclusion: Endotoxin, proinflammatory cytokines and anti-inflammatory cytokines play an important role in acute infection and can be used as an index to evaluate the severity of infectious diseases.
