目的探讨眼前部缺血性视神经病变(anterior ischemic optic neuropathy,AION)患者球后注射扩血管药物的必要性。方法对39例(39只眼)临床诊断为AION患者,分别于球后注射山莨菪碱前、注射15 min和1 h后,应用彩色多普勒血流显像技术检测患眼各项血流动力学指标。包括眼动脉、视网膜中央动脉、鼻和颞侧睫状后短动脉收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)、单位时间内平均最大血流速度(TAMX)及血流搏动指数(PI)、阻力指数(RI)。同时将上述检测指标进行比较。结果AION患眼与健眼相比,睫状后短动脉各项参数指标差异有统计学意义(P<0.01)。山莨菪碱球后注射15 min和1 h后,各血流参数指标除阻力指数外,均较注射前增高,差异有统计学意义(P<0.01)。结论睫状后短动脉和视网膜中央动脉检测结果表明,球后注射山莨菪碱15 min和1 h后,CDFI显示血流动力学参数指标变化明显。山莨菪碱球后注射可以有效改善鼻、颞侧睫状后短动脉及视网膜中央动脉的血流速度。
Objective To investigate the necessity of injecting vasodilators in patients with anterior ischemic optic neuropathy (AION). Methods Thirty-nine patients (39 eyes) were enrolled in this study. AION patients were treated with anisodamine injection before and 15 min and 1 h after injection. Color Doppler flow imaging was used to detect the blood flow Kinetic index. Including peak systolic velocity (PSV), end diastolic velocity (EDV), mean maximum velocity of blood flow per unit time (TAMX), and blood flow in the central retinal artery, central retinal artery, Beating index (PI), resistance index (RI). At the same time the above test indicators are compared. Results Compared with healthy eyes, AION eyes had statistically significant parameters (P <0.01). After 15 min and 1 h of retrobulbar injection of anisodamine, the indexes of blood flow parameters were all higher than those before injection except for the resistance index (P <0.01). Conclusions The posterior ciliary artery and the central retinal artery test results show that after the ball injection of anisodamine 15 min and 1 h after the ball, CDFI showed significant changes in hemodynamic parameters. Anisodamine injection of the ball can effectively improve the nasal and temporal ciliary short aorta and central retinal artery blood flow velocity.
