目的了解金黄色葡萄球菌携带杀白细胞素(PVL)编码基因pvl情况、流行特征及耐药性。方法 PCR检测金黄色葡萄球菌pvl基因,MIC仪器法检测菌株对13种临床常用抗菌药物的敏感性。结果 280株金黄色葡萄球菌中,48株(17.1%)携带pvl基因(pvl+),82株为耐甲氧西林金黄色葡萄球菌(MRSA),MRSA pvl的阳性率(19.5%)与MSSA(16.2%)相比,差异无统计学意义(χ~2=20.458,P=0.498)。各标本中以分离自脓液的金黄色葡萄球菌pvl基因阳性率最高(25.4%),其次为痰液(15.4%)、分泌物(14.4%)、血液(14.3%),尿液和穿刺液中未发现pvl+菌株。280株金黄色葡萄球菌对13种抗菌药物敏感试验结果显示,耐药率前3位的依次是青霉素(96.4%)、红霉素(58.6%)、克林霉素(51.4%),对其他抗菌药物耐药性均较低(<30%),未发现利奈胺烷和万古霉素耐药菌株。pvl+金黄色葡萄球菌与pvl-金黄色葡萄球菌相比,对所有抗菌药物的耐药率差异无统计学意义(P>0.05)。结论 pvl+金黄色葡萄球菌可在不同感染类型中流行,其中以皮肤与软组织感染为主,该类菌株致病性较强,应加强该类菌株的监测。
Objective To understand the prevalence, epidemiology and drug resistance of pvl gene encoding Staphylococcus aureus carrying leukotriene (PVL). Methods The pvl gene of Staphylococcus aureus was detected by PCR. The susceptibility of the strains to 13 commonly used antimicrobial agents was tested by MIC instrument. Results Among the 280 Staphylococcus aureus strains, 48 (17.1%) carried pvl gene (pvl +) and 82 strains were methicillin - resistant Staphylococcus aureus (MRSA). The positive rate of MRSA pvl was 19.5% %), The difference was not statistically significant (χ ~ 2 = 20.458, P = 0.498). The positive rate of pvl gene in S. aureus isolated from pus was highest (25.4%), followed by sputum (15.4%), secretions (14.4%), blood (14.3%), urine and puncture fluid No pvl + strains were found. The sensitivity tests of 280 strains of Staphylococcus aureus to 13 kinds of antibacterials showed that the top three strains were penicillin (96.4%), erythromycin (58.6%) and clindamycin (51.4%), followed by the other Antimicrobial resistance was low (<30%), and no lenalidomide and vancomycin resistant strains were found. Compared with pvl-Staphylococcus aureus, there was no significant difference in drug resistance rate between pvl + Staphylococcus aureus and all antibacterials (P> 0.05). Conclusion pvl + Staphylococcus aureus can be popular in different types of infection, of which the skin and soft tissue infections are predominant. The strains are highly pathogenic and the monitoring of these strains should be strengthened.
