目的探讨高频振荡机械通气(HFOV)治疗新生儿重症呼吸衰竭的疗效。方法将26例采用同步间歇指令通气+压力支持模式治疗失败的呼吸衰竭患儿改为HFOV治疗,比较HFOV治疗前后动脉血氧分压(PaO2)二氧化碳分压(Pa CO2)、吸入氧浓度(FiO2)、氧合指数(OI)的变化及临床疗效。结果治疗后2、6、24 h患儿PaCO2较治疗前逐步降低,PaO2逐步升高,FiO2逐步降低,OI逐步升高,组间数据对比差异有统计学意义(P<0.05)。结论 HFOV对常频治疗失败的新生儿重症呼吸衰竭患儿疗效显著。
Objective To investigate the effect of high frequency oscillatory mechanical ventilation (HFOV) on neonatal severe respiratory failure. Methods Twenty-six children with respiratory failure who failed to respond to synchronized intermittent mandatory ventilation and pressure support were enrolled in the study. The changes of PaCO 2, PaCO 2 and FiO 2 before and after HFOV were compared. ), Oxygenation index (OI) changes and clinical efficacy. Results PaCO2 in children at 2, 6, 24 h after treatment decreased gradually, PaO2 gradually increased, FiO2 decreased gradually, and OI increased gradually.
