目的观察老年不稳定心绞痛患者硝酸酯类制剂偏心性给药的有效性及安全性方法。方法观察年龄60～80岁不稳定心绞痛男性患者50人,按就诊顺序以1:1的方式将其随机编入莫诺确特组:硝酸甘油持续静脉泵入48小时后停药改口服莫诺确特50 mg于晨6点一次给药;苯磺酸氨氯地平组:硝酸甘油持续静脉泵入48小时后改为白天持续泵入8小时,睡前服用苯磺酸氨氯地平2.5～5 mg。观察2组患者停用硝酸甘油后24～72小时心绞痛发作、心电图及偏心性给药后第24、48、72小时0～3点血压、心率等情况并记录。结果在硝酸酯制剂偏心性治疗24～72小时后心绞痛的发作次数、时间,2组间无显著性差异(P>0.05);72小时六分钟步行试验心电图改善有效率比较,无显著性差异(P>0.05);偏心性给药后24、48、72小时0～3点收缩压、舒张压、心率之间比较,无显著性差异(P>0.05)。结论在对老年男性不稳定心绞痛患者进行的硝酸酯类制剂偏心性治疗,无论是莫诺确特50 mg/d给药,还是白天持续静脉滴注硝酸甘油8小时、睡前口服苯磺酸氨氯地平2.5～5mg,均安全、有效。
Objective To observe the efficacy and safety of nitrates in the elderly patients with unstable angina pectoris. Methods Fifty male patients with unstable angina at 60 to 80 years old were randomly divided into 1 group and 1 group according to the order of their visits. They were randomly assigned into monotherapy group. After continuous intravenous infusion of nitroglycerin for 48 hours, Indeed 50mg at 6:00 once the morning administration; amlodipine besylate group: nitroglycerin continuous intravenous infusion of 48 hours to pump continuously for 8 hours during the daytime, before taking amlodipine besylate 2.5 to 5 mg. The angina pectoris was observed 24 hours to 72 hours after stopping the administration of nitroglycerin, and the blood pressure and heart rate at 24, 48 and 72 hours after electrocardiogram and eccentric administration were recorded and recorded. Results There was no significant difference (P> 0.05) in the number and duration of angina pectoris after 24 to 72 hours of nitrocellulose preparation eccentricity treatment. Conclusions In the treatment of eccentric nitroglycerin in elderly patients with unstable angina, whether Monocrotal 50 mg / d, or continuous intravenous nitroglycerin during the day 8 hours, bedtime oral administration of benzene sulfonic acid Chlorpheniramine 2.5 ~ 5mg, are safe and effective.
