目的:比较运动中收缩压(SBP)的反应与冠心病(CHD)患者冠状动脉病变特点和血管活性物质(VS)变化.方法:对41例冠状动脉造影证实的CHD患者根据运动中SBP变化分为3组:下降至少10mmHg者(A组,n=12)、升高≤50mmHg者(B组,n=16)和显著升高>50mmHg者(C组,n=13),放免法测定运动前后血浆内皮素、血管紧张素、血栓素、前列腺素和血小板膜糖蛋白变化.结果:A组冠状动脉病变积分高、心功能差,运动前VS浓度高;C组运动后VS浓度明显升高.结论:A组冠状动脉病变严重,静息时VS已处较高水平代偿状态;C组VS对运动呈高敏易激活,与SBP显著升高有关.“,”Objective:To study on the relationship between different SBP responce to exerciseand lesions of coronary artery and vasoactive substance- Method: 41 patients with coronary arterydisease documented by angiography were divided into three groups based on the SBP response toexercise:a decrease of at least 10 mmHg (group A, n = 12);an increase of ≤5O mmHg (group B, n= l6) and an increase by more than 50 mmHg(group C, n = 13). Radioimmunoassay was used forthe determination of plasma level of endothelin (ET-1 ), angiotension (Ang Ⅱ ), thromboxane(TxB2,), prostaglandin (6-keto-PGF1α) and α- granule membrane protein (GMP- 14O) pre- and postexercise and compared with severity of coronary artery disease evaluated by a scoring system. Re-sult:The angiographic score was significantly higher in group A (30.6±12.2) compared with thatin group B (21.1±9. 7), P < 0.05 and group C (12.8±5.6), P <0.01. The abnormal function ofleft ventricle was more common in group A. The plasma level of ET-1, Ang Ⅱ,TxB2 and GMP-140 at rest was higher in group A than that in group B and group C, but the increased changes ofthose vasoactive substances with exercise were significantly higher in group C than that in group Aand group B(P<0.05~0.01). Conclusion: There were severe lesions of coronary artery and ele-vated p1asma level of vasoactive substance at rest in group A. Vasoactive substance was easily in-creased during exercise in group C,which was related to the exaggerated SBP response.
