目的 :研究早期原发性甲状旁腺机能亢进 (PHPT)的血清钙、磷、甲状旁腺素 (PTH)诊断价值。方法 :将 6 0只成年白兔随机分为 6组 ,每组 1 0只。实验组和对照组各 30只 ,对照组喂以正常饮食 (Ca∶P ,1∶0 .7) ,实验组喂以高磷饮食 (Ca∶P ,1∶7) ,建立原发性甲状旁腺机能亢进动物模型 ,在喂养前一周及喂养后第 1 ,2 ,3个月时 ,对所有动物做血钙、血磷、血PTH测定 ,并在第 1 ,2 ,3个月末时 ,将对照组与实验组动物各 1 0只处死 ,进行甲状旁腺及肾组织学检查 ,观察甲状旁腺及肾脏的组织学改变。结果 :实验组血PTH水平明显高于对照组 ,血钙水平明显低于对照组 ,而血磷水平变化无显著差异。甲状旁腺组织学检查 ,在 1个月时无变化 ,在 2个月时 ,实验组中的 1 0例中 6例显示轻度增生 ,3个月时 1 0例中 9例轻、中度增生。肾脏组织学检查 ,在 1个月时无变化 ,但在第 2、3个月时发现肾实质内炎性改变 ,并伴钙盐沉积。结论 :血清钙下降 ,血磷无明显变化 ,血PTH水平升高可能比高血钙、低血磷对PHPT的早期诊断更为重要
Objective: To investigate the diagnostic value of serum calcium, phosphorus and parathyroid hormone (PTH) in patients with early primary hyperparathyroidism (PHPT). Methods: Sixty adult white rabbits were randomly divided into 6 groups with 10 rats in each group. The experimental group and the control group each 30, the control group fed a normal diet (Ca: P, 1:07), the experimental group fed high phosphorus diet (Ca: P, 1: 7), the establishment of primary parathyroid Hyperthyroidism animal models, in the week before feeding and 1, 2, 3 months after feeding, all animals for blood calcium, phosphorus, blood PTH determination, and at the end of 1, 2, 3 months, will be The control group and experimental group of animals were sacrificed 10, parathyroid and renal histological examination, observation of parathyroid and kidney histological changes. Results: The blood PTH level in the experimental group was significantly higher than that in the control group, the serum calcium level was significantly lower than that in the control group, while the serum phosphorus level had no significant difference. There was no change in parathyroid histology at 1 month. At 2 months, 6 of 10 cases in the experimental group showed mild hyperplasia and 10 of 10 cases were mild and moderate at 3 months Hyperplasia. Renal histology did not change at 1 month, but inflammatory changes in the renal parenchyma were found at the 2nd and 3rd month with calcium deposition. CONCLUSIONS: Serum calcium decreased and there was no significant change in serum phosphorus. Elevated blood levels of PTH may be more important for the early diagnosis of PHPT than hypercalcemia and hypophosphatemia
