目的观察低浓度丝裂霉素C(mitomycin C,MMC)在翼状胬肉切除伴角膜缘干细胞移植术中应用的疗效及其对眼表的影响。方法将91例98眼原发性翼状胬肉随机分成两组,对照组45例48眼,行翼状胬肉切除伴角膜缘干细胞移植术;观察组46例50眼,行翼状胬肉切除伴角膜缘干细胞移植术联合MMC。术前及术后60 d均进行泪膜破裂时间(breakup time,BUT)和基础泪液分泌试验(shirmer I test,SIt)检查。结果术后60 d,两组患者BUT及SIt结果与术前相比未见明显差异。随访1年,对照组复发率35.4%;观察组复发率14.0%。两组复发率比较,差异有统计学意义(χ2=6.074,P=0.014)。结论在翼状胬肉切除伴角膜缘干细胞移植术中应用丝裂霉素C,能有效提高治愈率,降低复发率。低浓度的丝裂霉素C并未影响泪膜的稳定性,安全有效,值得临床推广。
Objective To observe the curative effect of low concentration mitomycin C (MMC) applied in limbal stem cell transplantation in patients with pterygium excision and its influence on ocular surface. Methods Ninety-one patients (98 eyes) with primary pterygium were randomly divided into two groups. The control group (45 eyes) and 48 eyes (45 eyes) underwent pterygium excision with limbal stem cell transplantation. The observation group consisted of 46 patients (50 eyes) with pterygium resection and cornea Marginal stem cell transplantation combined with MMC. The breakup time (BUT) and shirmer I test (SIt) were performed before and 60 days after operation. Results 60 days after operation, there was no significant difference in BUT and SIt between the two groups. Followed up for 1 year, the control group recurrence rate was 35.4%; observation group recurrence rate was 14.0%. The recurrence rate of the two groups, the difference was statistically significant (χ2 = 6.074, P = 0.014). Conclusion The application of mitomycin C in limbal stem cell transplantation in pterygium excision can effectively improve the cure rate and reduce the recurrence rate. Low concentrations of mitomycin C did not affect the stability of the tear film, safe and effective, worthy of clinical promotion.
