目前,许多医院把创收放在提高病床使用率和增加收费项目,广泛应用高新技术方面。但昆明医学院附一院则走出了一条抓效率,创效益的卫生经济改革路子,取得了社会效益与经济效益同步增长的效果。1993年度,昆明医学院附一院首先在外三科进行试点;此后扩大到心内科等8个科室。试点方案以降低出院者平均住院天数,提高病床周转次数为基础目标,辅以质量指标控制和包含设备折旧的支出成本核算。通过相当于90个标准床年的试验,达到了以下目标。出院者平均住院日降到20天以内,减少6.87天。收治病人数增加57%。增收病人的服务量相当于增设约40个标准病房。减少因住院损失劳动日13300天。人均医疗费用
At present, many hospitals place income generation on improving the utilization rate of hospital beds and increasing the number of charging items, and extensively apply high technology. However, the First Affiliated Hospital of Kunming Medical College has come out with a way to grasp the efficiency and create an effective health economic reform, and has achieved the simultaneous growth of social and economic benefits. In 1993, the First Affiliated Hospital of Kunming Medical College first conducted pilots in three external departments; thereafter, it expanded to eight departments including Cardiology. The pilot program is based on the goal of reducing the average length of hospital stay for hospital dischargers and increasing the number of hospital bed turnovers, supplemented by quality cost control and expenditure cost accounting for equipment depreciation. Through the experiment equivalent to 90 standard bed years, the following goals were achieved. The average length of hospital discharge for hospital dischargers dropped to less than 20 days, a decrease of 6.87 days. The number of patients admitted increased by 57%.
