连续性动静脉血液滤过对小儿急性肾功能衰竭治疗作用的观察

目的 观察连续性动静脉血液滤过对小儿急性肾功能衰竭的治疗作用。方法  3例采用连续动静脉血液滤过 (CAVH)及 1例采用连续静脉 -静脉血液滤过 (CVVH)替代传统血液透析疗法治疗不同原因所致急性肾功能衰竭患儿 4例。置换液使用乳酸林格液由动脉端输入。且肝素泵持续由动脉端泵入肝素维持肝素化抗凝。结果 平均滤过时间为 5 5h ,平均血流速度 47 1mL/min ,平均滤过分数 0 199,平均滤过率为 2 5mL/min。滤过后水肿均减轻或消失 ,血BUN及Cr分别由滤过前的 2 7 5mmol/L、46 8 8μmol/L降至滤过后的 13 0mmol/L、16 9 9μmol/L ,代谢性酸中毒及电解质失衡也得到纠正 ,3例患儿恢复排尿并痊愈出院 ,1例患儿入院 10天一直无尿自动出院。结论 CAVH是治疗小儿急性肾功能衰竭的简单、安全、有效的方法。 Objective To observe the therapeutic effect of continuous arteriovenous hemofiltration on acute renal failure in children. Methods Four patients with acute renal failure due to different causes were treated with continuous arteriovenous hemofiltration (CAVH) and one with continuous venovenous hemofiltration (CVVH) instead of conventional hemodialysis. Replacement fluid using lactated Ringer’s solution input from the arterial side. And the heparin pump continues to pump heparin from the arterial side to maintain heparinized anticoagulation. Results The average filtration time was 55h, the average blood flow 47 1mL / min, the average filtration fraction 0 199, the average filtration rate of 25mL / min. The edema of blood was reduced or disappeared after filtration, the blood BUN and Cr decreased from 270 mmol / L and 468 8 μmol / L before filtration to 13 0 mmol / L and 16 9 9 μmol / L after filtration, respectively. Metabolic acidosis and Electrolyte imbalances were also corrected. Three children recovered from urination and were cured. One patient had been discharged without urine after 10 days of admission. Conclusion CAVH is a simple, safe and effective method for the treatment of children with acute renal failure.
