目的:探讨心功能对心肌梗死犬心肌血流灌注的影响。方法:18只健康杂种犬于前降支分出的第1对角支远端约1cm处结扎3h,应用心肌超声造影(MCE)定量分析左室前壁中间段和下壁中间段心肌血流量(MBF)。结果:17只犬成功建立急性心肌梗死模型。根据结扎3h后左室整体射血分数(EF)分为2组:A组(EF≥50%)7只,B组(EF<50%)10只。B组左室前壁中间段和下壁中间段MBF均低于A组,但2组之间差异无统计学意义;与结扎前相比,2组左室前壁中间段MBF均明显降低,差异有统计学意义(P<0.05),A组左室下壁中间段MBF略升高,B组则降低,但均差异无统计学意义。结论:心功能对MBF有一定的影响,可能会低估冠状动脉血流储备和高估狭窄程度,在以MCE诊断冠心病时应注意心功能对MBF的影响。
Objective: To investigate the influence of cardiac function on myocardial perfusion in dogs with myocardial infarction. Methods: Eighteen healthy mongrel dogs were ligated for 3 hours at the distal end of the first diagonal branch of the anterior descending artery for 3 hours. Myocardial contrast echocardiography (MCE) was used to quantitatively analyze myocardial blood flow in the middle and anterior segments of the left anterior wall (MBF). Results: 17 dogs successfully established acute myocardial infarction model. According to the total ejection fraction (EF) of left ventricle after 3 hours of ligation, there were 2 groups: 7 in group A (EF≥50%) and 10 in group B (EF <50%). MBF in the middle segment of the left anterior wall and the middle segment of the inferior wall in group B were lower than those in group A, but there was no significant difference between the two groups. MBF in the middle segment of left anterior wall was significantly lower in both groups The difference was statistically significant (P <0.05). In group A, the MBF in the middle part of the left subventricular wall increased slightly, while in group B, the MBF decreased slightly, but the difference was not statistically significant. CONCLUSION: Cardiac function has certain influence on MBF, which may underestimate coronary blood flow reserve and overestimate the degree of stenosis. In the diagnosis of coronary heart disease by MCE, the effect of cardiac function on MBF should be noticed.
