目的探讨男性后尿道狭窄患者在导尿困难时应用不锈钢导尿管支架导尿的操作要点及临床价值。方法回顾性分析总结32例男性后尿道狭窄患者在导尿困难时应用不锈钢导尿管支架治疗的经验。结果 28例导尿获得成功,4例导尿失败,导尿成功率为87.5%。导尿成功的病人1~2周后拔除导尿管,18例排尿通畅;其余10例行钬激光联合电切治疗,术后1周拔除导尿管,均排尿通畅,予定期行尿道扩张治疗。随访6~12个月,无明显排尿困难,平均最大尿流率16.5(11~20)ml/s。结论男性后尿道狭窄患者在导尿困难时应用不锈钢导尿管支架做支撑导尿,成功率高,操作简单,避免了膀胱造瘘,部分病人可以无需手术治疗。
Objective To investigate the operative points and clinical value of catheterization in patients with posterior urethral stricture using stainless steel catheter in difficult urethral catheterization. Methods A retrospective analysis of 32 cases of posterior urethral stricture in patients with difficult catheterization of stainless steel catheter stent treatment experience. Results 28 cases of successful catheterization, 4 cases of catheterization failed, catheterization success rate was 87.5%. One to two weeks after successful catheterization, the catheter was removed and 18 cases of urination were unobstructed. The remaining 10 cases were treated with holmium laser and electrotomy. One week after the operation, the catheter was removed and voiding was unobstructed. Urethral dilation was performed regularly . Follow-up 6 to 12 months, no significant dysuria, the average maximum flow rate of 16.5 (11 ~ 20) ml / s. Conclusions In patients with posterior urethral stricture, ureteral stents are used to support urinary catheterization in difficult urethral catheterization. The success rate is high, the operation is simple, bladder fistula is avoided, and some patients can be treated without surgery.
