目的:根据CT扫描图像及三维重建表现,探讨多层螺旋CT扫描对小肾癌的诊断及鉴别诊断的价值,并对小肾癌进行分型。资料与方法:收集行多层螺旋CT扫描并经临床病理证实的43例小肾癌病例资料,就其CT表现进行分析并进行分型。结果:凸出型小肾癌23例,CT表现为肿瘤凸向肾轮廓之外或凸向肾盂、肾盏,增强扫描时,肿瘤内部造影剂呈明显快进快出的典型恶性肿瘤表现;肾实质内型小肾癌6例,CT表现为肿瘤位于肾实质之内,增强扫描肿瘤呈典型肾癌强化特征;囊性型小肾癌9例,CT表现为肿瘤内部有多发纤维索条状或小片状强化,且强化程度较明显;多发型小肾癌5例,CT表现为一侧肾脏出现多个小肾癌。结论:多层螺旋CT扫描结合三维重建图像后处理技术能够准确、有效地诊断各种类型的小肾癌。
Objective: To evaluate the value of multi-slice spiral CT in the diagnosis and differential diagnosis of small renal cell carcinoma based on CT and 3D reconstruction. Materials and Methods: The data of 43 cases of small renal cell carcinoma confirmed by clinicopathology were collected and analyzed by multi-slice spiral CT. Their CT findings were analyzed and classified. Results: 23 cases of convex small renal cell carcinoma, CT showed the tumor convex to the renal outline or convex to the renal pelvis, calyx, enhanced scan, the tumor contrast agent was significantly faster than the typical tumor performance; kidney 6 cases of endogenous small renal cell carcinoma, CT showed that the tumor was located within the renal parenchyma, enhanced scan of the tumor showed typical features of renal cell carcinoma; Cystic small renal cell carcinoma in 9 cases, CT showed multiple tumor within the fiber cable or Small patch-like enhancement, and the degree of enhancement is more obvious; multiple small renal cell carcinoma in 5 cases, CT showed one side of the kidney appeared multiple small renal cell carcinoma. CONCLUSIONS: Multi-slice spiral CT combined with three-dimensional reconstructed image postprocessing can accurately and effectively diagnose various types of small renal cell carcinoma.
