目的:观察催产素联合止血敏不同剂量用于剖宫产术后对子宫复旧、宫腔内容物排出、子宫切口愈合情况的影响,降低剖宫产术后并发症的发生。方法:将2005年房山区妇幼保健院剖宫产术后产妇340例随机分为两组,观察组171例,对照组169例。观察组手术当天使用催产素30U,止血敏1000mg静点;术后1~3天每天催产素20U,止血敏1000mg静点。对照组手术当天催产素20U,止血敏500mg静点,术后1~3天每天催产素10U,止血敏500mg静点。手术后第3天B超监测子宫复旧、宫腔内容物排出、子宫切口愈合情况。统计学处理采用χ2检验。结果:B超监测340例剖宫产术后病例,发生异常38例,异常率11.18%。其中观察组异常10例,异常率5.85%;对照组异常28例,异常率16.57%。两组比较差异有极显著性意义(P<0.005)。结论:剖宫产术后大剂量应用催产素和止血敏,促进子宫复旧,降低剖宫产术后并发症的发生率,预防产后出血,提高产科质量。
OBJECTIVE: To observe the effect of oxytocin combined with hemostatic sensitivity on cesarean section after cesarean section on uterine involution, uterine cavity contents excretion and uterine incision healing, and to reduce the incidence of complications after cesarean section. Methods: 340 cases of maternal postpartum cesarean section in Fangshan District MCH hospital in 2005 were randomly divided into two groups: 171 cases in observation group and 169 cases in control group. The observation group on the day of surgery with oxytocin 30U, hemostatic sensitivity 1000mg static point; 1 to 3 days after surgery oxytocin 20U daily, hemostatic 1000mg static point. On the day of surgery, oxytocin 20U and hemostatic 500mg were administered in the control group. Oxytocin 10U and hemostatic 500mg were intravenously administered 1 to 3 days after operation respectively. B-monitoring of uterine involution, uterine cavity contents, uterine incision healing on the third day after surgery. Statistical analysis using χ2 test. Results: 340 cases of post-cesarean section were monitored by B-mode ultrasonography and 38 cases were abnormal, with an abnormal rate of 11.18%. Among them, 10 cases were abnormal in the observation group and the rate of abnormality was 5.85%. In the control group, 28 cases were abnormal, and the abnormal rate was 16.57%. There was significant difference between the two groups (P <0.005). Conclusion: After cesarean section, oxytocin and hemostatic sensitization are given in large doses to promote uterine involution and reduce the incidence of complications after cesarean section, prevent postpartum hemorrhage and improve the quality of obstetrics.
