目的 :明确FHIT基因蛋白产物 (Fhit)细胞内定位及分布情况 ,比较其在肾细胞癌 (RCC)组织、癌周及正常肾组织中的表达差异 ,探讨Fhit表达状态与RCC临床病理特征的关系。方法 :采用兔抗 Fhit多克隆抗体对 36例RCC的癌组织、2 0份相应癌周肾组织及 6例正常肾组织标本行免疫组织化学ABC法染色 ,结果以半定量方法记录分析。结果 :RCC患者术后五年随访过程中 ,复发及转移发生率分别为 16 7% (6 /36 )和 7 7% (10 /36 )。五年生存率为 5 5 6 % (2 0 /36 )。正常肾组织中特异性着色区主要分布在肾小管上皮细胞胞浆 ,呈弥漫性表达 ,少量位于肾小球内。癌组织中特异性着色区位于癌细胞胞浆 ,Fhit阳性率为 5 2 8% (19/36 ) ,癌组织中Fhit表达强度明显低于正常肾脏组织和相应癌周肾组织 (P <0 0 1)。Ⅲ、Ⅳ期癌组织Fhit表达显著低于Ⅰ、Ⅱ期者 (P <0 0 5 )。RCC组织Fhit失表达与患者预后差关系密切 (P <0 0 5 )。结论 :Fhit为一种细胞浆蛋白 ,正常肾组织中呈强阳性表达 ,绝大部分位于肾小管上皮细胞。癌组织中Fhit表达显著降低或失表达。其表达情况可提示RCC患者预后
OBJECTIVE: To determine the intracellular localization and distribution of FHIT gene product (Fhit), compare the expression of Fhit in renal cell carcinoma (RCC), pericancerous tissue and normal renal tissue, and to explore the relationship between Fhit expression and clinicopathological features of RCC . Methods: The anti-Fhit polyclonal antibody was used to stain immunohistochemical ABC method in 36 cases of RCC, 20 cases of corresponding peritumoral kidney and 6 cases of normal renal tissues. The results were recorded and analyzed by semi-quantitative method. Results: The rates of recurrence and metastasis in patients with RCC were 16.7% (6/36) and 77% (10/36), respectively. The five-year survival rate was 55.6% (20/36). The specific staining area in normal kidney tissue is mainly distributed in the cytoplasm of renal tubular epithelial cells, showing diffuse expression, a small amount located in glomeruli. The positive expression rate of Fhit was 52.8% (19/36) in the cytoplasm of cancer cells, and the expression of Fhit in cancer tissues was significantly lower than that in normal kidney tissues and corresponding peritumoral kidney tissues (P <0 0 1). The expression of Fhit in stage Ⅲ and Ⅳ cancers was significantly lower than that in stage Ⅰ and Ⅱ (P <0.05). The expression of Fhit loss in RCC was closely related to the prognosis of patients (P <0.05). Conclusion: Fhit is a kind of cytoplasm protein, which is strongly expressed in normal kidney tissues, mostly in renal tubular epithelial cells. Fhit expression was significantly decreased or overexpressed in cancerous tissues. Its expression can prompt the prognosis of patients with RCC
