目的 探讨妊娠期高血压病(妊娠高血压与先兆子痫)与日后发生循环系统疾病之间的关系。 设计 首次单胎妊娠期间发生先兆子痫妇女的队列研究。两比较组之间年龄、分娩时年龄相匹配,其中一组患有妊娠高血压,另一组没有血压升高的病史。 地点 苏格兰Grampian区的产院。 参与者 从Aberdeen产院1951年至1970年的产妇与新生儿数据库中挑选出的妇女,这些妇女都居住在Aberdeen市,并且都是头胎单产。 主要结果测量方法 当前的生存状况及心血管健康状况。通过信访问卷调查、临床体检、出院记录及死亡资料确定。 结果 在所有的测量中,先兆子痫/子痫或者妊娠高血压与日后发生高血压都有显著的正相关。对妊娠高血压,其调整的相对危险为1.13~3.72不等,对先兆子痫或者子痫调整的相对危险为1.40~3.98不等。先兆子痫/子痫组因卒中而死亡的发生率比数比是3.59(95%的可信区间是1.04-12.4)。 结论 妊娠期高血压病与日后发生高血压相关疾病有关。如果更加注意这种相关性,做到早期诊断与有效治疗,可能在一定范围内减少由于这种疾病所导致的致残和死亡比例。
Objective To investigate the relationship between hypertensive disorders of pregnancy (pregnancy-induced hypertension and preeclampsia) and the occurrence of circulatory diseases in the future. A cohort study was designed to designate women with preeclampsia during the first singleton pregnancy. The age of the two comparisons, the age at childbirth, matched one group with gestational hypertension and the other without a history of elevated blood pressure. Place of birth in Grampian district, Scotland. Participants were selected from the Maternal and Neonatal Database of Aberdeen Homes from 1951 to 1970 and all resided in Aberdeen and were both first-born. The main outcome measures the current living conditions and cardiovascular health status. Through the petition questionnaire survey, clinical examination, discharge records and death information. Results In all measurements, preeclampsia / eclampsia or gestational hypertension had a significant positive correlation with future hypertension. For pregnancy-induced hypertension, the relative risk of adjustment ranging from 1.13 to 3.72, the relative risk of pre-eclampsia or eclampsia adjusted for 1.40 to 3.98 range. The odds ratio for stroke death from preeclampsia / eclampsia was 3.59 (95% confidence interval, 1.04-12.4). Conclusions Hypertensive disorder in pregnancy is associated with hypertension-related diseases in the future. If we pay more attention to this correlation and achieve early diagnosis and effective treatment, we may reduce the proportion of disability and death caused by this disease to a certain extent.
