【目的】观察铁剂在儿童反复呼吸道感染辅助治疗中的作用。【方法】我院儿科2~3岁反复呼吸道感染患儿83例,随机分配进入治疗组和对照组。两组患儿均按常规抗感染治疗,治疗组加用小儿硫酸亚铁糖浆辅助治疗,按元素铁2~6mg/kg.d,每3天1次口服,连续12周。两组均于治疗前及治疗后12个月抽血,应用流式细胞术检测淋巴细胞亚群CD3+,CD4+,CD8+,计算CD4+/CD8+比值,同时测量患儿的血红蛋白和血清铁蛋白含量等铁营养状况指标,记录呼吸道感染情况。【结果】铁剂治疗后12个月治疗组患儿的铁营养状况指标值均高于对照组;铁剂治疗后12个月与治疗前相比淋巴细胞亚群CD3+,CD4+,CD8+及CD4+/CD8+比值的差值治疗组均高于对照组;铁剂治疗后12个月有效率治疗组高于对照组;两者差异均有统计学意义。治疗组患儿总呼吸道感染次数较对照组明显减少,再次呼吸道感染时病程缩短,病情减轻。【结论】加铁剂辅助治疗能明显提高反复呼吸道感染患儿的细胞免疫功能;适量补充铁剂有助于预防小儿反复呼吸道感染。
【Objective】 To observe the role of iron in adjuvant therapy of recurrent respiratory tract infections in children. 【Methods】 83 children with recurrent respiratory tract infections aged 2 to 3 years old in our hospital were randomly assigned to treatment group and control group. The two groups of children were routinely treated with anti-infective therapy. The treatment group was supplemented with pediatric ferrous sulfate syrup supplemented with 2 ~ 6 mg / kg.d of iron every 1 day for 12 weeks. Blood samples were collected before treatment and 12 months after treatment. The lymphocyte subsets CD3 +, CD4 + and CD8 + were measured by flow cytometry. The ratios of CD4 + / CD8 + were calculated. The contents of hemoglobin and serum ferritin were also measured Nutritional status indicators, record of respiratory infections. 【Results】 The iron nutrition status of the treatment group was higher than that of the control group at 12 months after treatment with iron. The levels of CD3 +, CD4 +, CD8 + and CD4 + / CD8 + CD8 + ratio of the treatment group were higher than the control group; 12 months after treatment with iron in the treatment group was higher than the control group; both differences were statistically significant. The total number of respiratory tract infections in children in the treatment group was significantly lower than that in the control group, and the course of disease was shortened and the disease was relieved again when the respiratory tract infection was repeated. 【Conclusion】 Iron-supplemented adjuvant therapy can significantly improve the cellular immune function in children with recurrent respiratory tract infection; moderate iron supplementation can help prevent recurrent respiratory tract infections in children.
