目的:探讨超声心动图各参数指标与N末端脑钠肽前体(NT-proBNP)在舒张功能不全的心房颤动(房颤)患者中的变化及其相关性。方法:选取舒张功能不全的房颤患者204例,分为阵发性房颤组87例,持续性房颤组117例(包含长期持续性和永久性房颤),另选取收治入院的100例不伴有房颤的舒张功能不全患者作为对照组,收集所有入选对象的超声心动图参数及NT-proBNP水平,并探讨其相关性。结果:房颤组患者的左心房内径、左心房容积指数、NT-proBNP均高于对照组,差异有统计学意义(P<0.05),且持续性房颤组与阵发性房颤组同样有统计学差异(P<0.05)。相关性分析显示,NT-proBNP与左心房内径(r=0.626,P<0.001)、左心房容积指数(r=0.470,P=0.001)呈正相关。结论:舒张功能不全的房颤患者左心房明显增大,NT-proBNP水平明显增高且与左心房内径、左心房容积指数相关。
Objective: To investigate the changes of echocardiographic parameters and NT-proBNP in patients with atrial fibrillation (AF) and its correlation with diastolic dysfunction. Methods: Totally 204 patients with atrial fibrillation with diastolic dysfunction were divided into 87 cases of paroxysmal atrial fibrillation group, 117 cases of persistent atrial fibrillation group (including long-term persistent and permanent atrial fibrillation), and another 100 cases admitted to hospital The patients with diastolic dysfunction who were not accompanied by atrial fibrillation were selected as the control group, echocardiographic parameters and NT-proBNP levels were collected and their correlations were also explored. Results: The left atrial diameter, left atrium volume index and NT-proBNP in patients with atrial fibrillation were significantly higher than those in control group (P <0.05), and were similar in patients with persistent AF and paroxysmal atrial fibrillation There was a significant difference (P <0.05). Correlation analysis showed that NT-proBNP was positively correlated with left atrial diameter (r = 0.626, P <0.001) and left atrial volume index (r = 0.470, P = 0.001). Conclusion: Atrial fibrillation patients with diastolic dysfunction significantly increased left atrium, NT-proBNP levels were significantly higher and with left atrial diameter, left atrial volume index.
