目的 观察乌拉地尔 (URA)对高血压患者血浆 4种神经肽 :内皮素 (ET)、降钙素基因相关肽 (CGRP)、神经肽Y(NPY)及神经降压素 (NT)水平的影响。方法 选择麻醉前高血压患者 2 0例。于麻醉诱导前静脉注射URA 0 .5mg/kg。应用放射免疫方法测定URA治疗前后血浆 4种神经肽的水平 ,并以 2 0例健康者作对照。结果 高血压组血浆ET、NPY水平明显高于正常组 (P <0 0 1) ,血浆CGRP、NT水平明显低于正常组 (P <0 0 1)。静脉注射URA后 5min ,高血压组血浆ET显著性降低且接近对照水平 (P <0 0 1)、NPY较治疗前显著性降低且明显低于对照水平 (P <0 0 1,P<0 0 5 ) ;血浆CGRP较治疗前显著性升高且达对照水平 (P <0 0 1)、NT虽较治疗前显著性升高但仍低于对照水平 (P <0 0 1,P <0 0 5 )。结论 内皮素、降钙素基因相关肽、神经肽Y及神经降压素的比例失衡与高血压的发生密切相关 ,URA可改善其平衡关系 ,可作为其降压机制之一。
Objective To investigate the effects of urapidil on plasma levels of four neuropeptides: endothelin (ET), calcitonin gene related peptide (CGRP), neuropeptide Y (NPY) and neurotensin (NT) in patients with essential hypertension influences. Methods Twenty patients with prehospital hypertension were selected. URA 0.5 mg / kg was given intravenously before induction of anesthesia. Radioimmunoassay was used to measure the plasma levels of four neuropeptides before and after URA treatment. Twenty healthy controls were used as controls. Results The levels of plasma ET and NPY in hypertension group were significantly higher than those in normal group (P <0.01). The levels of CGRP and NT in plasma were significantly lower than those in normal group (P <0.01). At 5 minutes after intravenous injection of URA, plasma ET was significantly lower in hypertension group and close to the control level (P <0.01). NPY decreased significantly and was significantly lower than that in control group (P <0.01, P <0 0 5). Plasma CGRP increased significantly and reached the level of control before treatment (P <0.01). NT was significantly higher than that before treatment but still lower than that of control (P <0.01, P <0 0) 5). Conclusions The imbalanced ratio of endothelin, calcitonin gene related peptide, neuropeptide Y and neurotensin is closely related to the occurrence of hypertension. URA can improve its balance and may be used as one of its antihypertensive mechanisms.
