[目的]观察养心汤联合西药治疗气虚血瘀型急性心肌梗死皮冠状动脉内介入术后疗效。[方法]使用随机平行对照方法,将148例住院及门诊患者按抛硬币法简单随机分为两组。对照组74例冠脉扩张剂、抗凝剂、硝酸甘油,抗心律失常和镇痛治疗。治疗组74例养心汤(党参、郁金、五味子、陈皮、川芎、桂枝、甘草炙、元胡各10g,菖蒲、丹参、山萸肉、玉竹、何首乌、葛根各15g,砂仁、麦冬、檀香各20g),1剂/d,水煎400m L,早晚口服,200m L/次;西药治疗同对照组。连续治疗90d为1疗程。观测临床症状、症候积分、LVEF、LVEDD、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈12例,显效15例,有效16例,无效5例,总有效率93.24%。对照组痊愈12例,显效23例,有效24例,无效22例,总有效率70.27%。治疗组疗效优于对照组(P<0.01)。证候积分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。心功能指标治疗组改善优于对照组(P<0.01)。[结论]养心汤联合西药治疗气虚血瘀型急性心肌梗死皮冠状动脉内介入术后疗效满意,无严重不良反应,值得推广。
[Objective] To observe the effect of Yangxin Decoction combined with western medicine on the treatment of acute myocardial infarction with coronary heart disease after Qi-deficiency and blood-stasis type acute myocardial infarction. [Methods] Using randomized parallel control method, 148 inpatients and outpatients were randomly divided into two groups according to the coin-tossing method. Control group of 74 cases of coronary dilators, anticoagulants, nitroglycerin, anti-arrhythmia and analgesic treatment. Treatment group, 74 cases of Yang Xin Tang (Codonopsis, Turmeric, Schisandra, dried tangerine peel, Chuanxiong, Guizhi, licorice Sunburn, Yuanhu each 10g, iris, Salvia, cornus, Polygonum, Radix, Pueraria each 15g, Amomum, Ophiopogon, sandalwood each 20g), 1 / d, decoction 400m L, morning and evening oral, 200m L / time; western medicine treatment with the control group. Continuous treatment of 90d for a course of treatment. Observation of clinical symptoms, symptom scores, LVEF, LVEDD, adverse reactions. Treatment of a course of treatment to determine the efficacy. [Results] The treatment group cured 12 cases, markedly effective in 15 cases, effective in 16 cases, ineffective in 5 cases, the total effective rate was 93.24%. The control group cured 12 cases, markedly effective in 23 cases, effective in 24 cases, ineffective in 22 cases, the total effective rate was 70.27%. The treatment group than the control group (P <0.01). Syndrome points were improved in both groups (P <0.01), treatment group improved better than the control group (P <0.01). Cardiac function improvement in the treatment group was better than that in the control group (P <0.01). [Conclusion] Yangxin Decoction combined with western medicine has satisfactory curative effect after intracoronary interventional therapy of acute myocardial infarction with qi deficiency and blood stasis type, without serious adverse reactions, which is worth popularizing.
