目的通过评价3种手套与4种消毒剂组合的个体效应,为大型医疗机构连续静脉采血人员在工作量大和时间不足情况下提供手卫生策略。方法以若干门诊静脉采血护士为观察对象,在对患者进行连续静脉采血操作中,用快速手消剂对手套进行消毒,监测手套表面的菌落数,比较分析手套/消毒剂的十二种组合类型,每种组合采样30份(共采样360份),并通过手套是否泄漏(共收集手套1392只)、消毒时间和手套物理性状的改变来综合比较分析。结果从消毒效果看,360份样本总体合格率达97.5%(合格样本351份),十二种组合的消毒效果差异无统计学意义;1392只手套中231只发生泄漏,手套泄露率差异有统计学意义(P<0.05),主要与3种手套间材质的不同有关(P<0.01),且随着连续操作次数的增加,手套的泄露呈上升趋势,其差异有统计学意义(P<0.05),手套的泄露部位主要位于拇指与食指指尖(54.5%)。结论在连续采血过程中,快速手消剂进行手套外消毒是安全有效、合理可行的;各医疗机构可结合自身情况,从安全、经济、省时等角度综合考虑,选择合适的手套/消毒剂组合。
Objective To assess the impact of the combination of three gloves and four disinfectants on hand-held strategies for large venous blood donors in large medical institutions with heavy workload and lack of time. Methods A number of outpatient venous blood sampling nurses were observed. During continuous venous blood sampling, patients were disinfected with rapid hand sanitizer and the number of colonies on the surface of the gloves was monitored. Twelve combinations of gloves and disinfectants were analyzed , 30 samples for each combination (360 samples in total), and through the glove leakage (a total of 1392 gloves collected), disinfection time and glove physical changes to a comprehensive comparative analysis. Results From the disinfection effect, the overall pass rate of 360 samples was 97.5% (351 qualified samples), there was no significant difference in the disinfection effect among the 12 combinations; 231 leaks occurred in 1392 gloves, there was statistical difference in the glove leakage rate (P <0.05), which was mainly related to the differences of materials between the three kinds of gloves (P <0.01). With the increase of the number of continuous operation, the leakage of gloves showed an upward trend with a significant difference (P <0.05 ), The leak area of the glove is mainly located at the tip of the thumb and index finger (54.5%). Conclusion During continuous blood sampling, extinction of gloves with gloves and disinfectants is safe, effective and feasible. All medical institutions can combine their own situation, consider the safety, economy and time-saving options, select the appropriate gloves / disinfectants combination.
