目的:探讨中医手法复位加上肢皮牵引悬吊及五合一夹板(GartlandⅢ型)配合中药口服治疗肱骨髁上骨折的治疗效果。方法:72例肱骨髁上骨折患者,随机分为对照组与治疗组各36例。对照组采用传统的手法整复及四合一小夹板外固定,治疗组采用手法整复、患肢皮肤牵引悬吊、五合一夹板外固定。对两组患儿的肘内翻、肘关节功能和临床疗效等相关指标进行比较。结果:治疗组与对照组优良率分别为97.2%、91.6%,组间比较,差异有统计学意义(P<0.05)。肘内翻发生情况为肘内翻≥10°,对照组为3例,治疗组为1例;肘内翻角度<10°,对照组为11例,治疗组为5例,两组的肘内翻发生情况治疗组均优于对照组(P<0.05)。结论:上肢皮牵引悬吊加五合一夹板固定治疗肱骨髁上骨折较传统小夹板外固定疗效好。
Objective: To investigate the therapeutic effect of traditional Chinese medicine reduction combined with traction and suspension of the limbs and a five-in-one splint (Gartland Ⅲ type) in combination with traditional Chinese medicine in the treatment of supracondylar humerus fractures. Methods: 72 patients with supracondylar humerus fractures were randomly divided into control group and treatment group, 36 cases each. The control group was treated by traditional method and four-in-one mini splint external fixation. The treatment group was treated by manual manipulation, limb traction and suspension, and external fixation of the splint. The two groups of children with cubitus varus, elbow function and clinical efficacy and other related indicators were compared. Results: The excellent and good rates of the treatment group and the control group were 97.2% and 91.6%, respectively. The differences between the two groups were statistically significant (P <0.05). The incidence of cubitus varus ≥10 ° elbow varus, the control group was 3 cases, the treatment group was 1 case; cubital valgus angle <10 °, the control group was 11 cases, the treatment group was 5 cases, the two groups within the elbow The incidence of turning over in treatment group was better than that in control group (P <0.05). CONCLUSION: The treatment of upper humeral supracondylar fractures with upper splint traction suspension and five-in-one splint fixation is more effective than the traditional small splint external fixation.
