目的 CT能谱成像(GSI)技术对鼻咽癌诊断的临床价值。资料与方法选取经鼻咽活检病理证实的92例鼻咽部病变患者(其中35例鼻咽癌为实验组,57例鼻咽炎症为对照组)。采用能谱CT对所有患者进行GSI单能模式增强扫描,测量并计算鼻咽部病变组织的碘浓度和碘浓度比,并与病理结果对照,计算能谱CT对鼻咽癌诊断的准确率、敏感性及特异性。结果实验组与对照组的碘浓度(100μg/m1)分别为,动脉期:13.16±3.23、9.69±2.86和静脉期:20.53±4.14、13.49±3.67,碘浓度比分别为动脉期:0.145±0.04、0.104±0.03和静脉期0.329±0.12、0.232±0.08。在动脉期,实验组与对照组的碘浓度和碘浓度比两两比较差异有统计学意义(P均<0.001);静脉期时,实验组与对照组的碘浓度和碘浓度比两两比较差异有统计学意义(P均<0.001)。能谱CT对鼻咽癌诊断的准确率、敏感性及特异性分别为95.65%、94.29%及96.49%。结论能谱CT对鼻咽癌的诊断具有重要价值。
Objective To evaluate the clinical value of CT spectral imaging (GSI) technique in the diagnosis of nasopharyngeal carcinoma. Materials and Methods 92 patients with nasopharyngeal lesions confirmed by pathology of nasopharyngeal biopsy were selected (35 nasopharyngeal carcinomas as experimental group and 57 nasopharyngeal inflammations as control group). The GSI single-energy mode enhanced scan was performed on all patients using energy spectrum CT. The iodine concentration and iodine concentration ratio of the nasopharyngeal lesion tissue were measured and compared with the pathological results. The diagnostic accuracy of the energy spectrum CT on nasopharyngeal carcinoma was calculated. Sensitivity and specificity. Results The concentrations of iodine (100μg/m1) in experimental group and control group were 13.16±3.23, 9.69±2.86 in arterial phase, 20.53±4.14 in venous phase, 13.49±3.67 in venous phase, and the iodine concentration ratio was 0.145±0.04 in arterial phase respectively. 0.104 ± 0.03 and venous phase 0.329 ± 0.12, 0.232 ± 0.08. In the arterial phase, the iodine concentration and the iodine concentration ratio of the experimental group and the control group had statistically significant differences (P<0.001 for both); in the venous phase, the iodine concentration and the iodine concentration ratio of the experimental group and the control group were compared. The difference was statistically significant (P all < 0.001). The accuracy, sensitivity and specificity of spectral CT in the diagnosis of nasopharyngeal carcinoma were 95.65%, 94.29% and 96.49%, respectively. Conclusion Energy spectrum CT has important value in the diagnosis of nasopharyngeal carcinoma.
