对33例有第三磨牙异常的颅颌功能紊乱患者进行了、髁突位置、TMJ上腔造影及翼外肌上下头肌电检查。结果表明,髁突移位与第三磨牙异常有明显的关系,其关节弹响的主要机理可能是由紊乱导致髁突移位,继之出现盘-突关系不调所致。而另一些患者髁突移位出现在弹响对侧关节,其翼外肌上下头电位异常且类似于弹响侧翼外肌功能紊乱及非弹响但有明显疼痛者的翼外肌电位。作者认为,翼外肌功能紊乱可以是上述颅颌功能紊乱的早期病变,是导致弹响增强的另一途径。异常导致下颌运动异常可能是其主要原因。
Thirty-three patients with craniofacial dysfunction who had a third molar abnormality were examined for condylar position, TMJ superior vena cava, and supraspinal myoelectricity. The results showed that there was a clear relationship between the condylar shift and abnormal third molars. The main mechanism of joint snapping may be the dislocation of the condyles caused by the disorder, followed by the discordance of the disc-protrusion relationship. In other patients, the condylar shift occurs in the contralateral joint of the snapping ring. The anteroposterior electrical potential of the pterygoid muscle is abnormal and resembles the pterygium potentials of the sylvian lateral wing muscle dysfunction and non-snapping but obvious pain. The authors believe that extra-vertebral muscle dysfunction can be an early lesion of craniofacial dysfunction and is another way to increase snapping. Abnormalities that cause abnormal mandibular movement may be the main reason.
