目的探讨MSCT肝动脉期血管成像技术对原发性肝癌肝外动脉供血情况的诊断和临床应用价值。方法对原发性肝癌行MSCT三期动态扫描,除行常规轴位及MPR观察外,特别行肝动脉系统三维血管重建,重建方法包括最大强度投影(MIP)、容积再现重建(VR)和表面遮盖成像(SSD)等。结果35例患者共发现39支肝外供血动脉。肝外供血动脉来源类型分别源于胸廓内动脉(5.1%),右肋间动脉(7.7%),胃左动脉(12.8%),右膈下动脉(38.5%),大网膜动脉(2.6%),右肾上腺或肾包膜动脉(10.3%)。结论MSCT肝动脉系统血管三维重建技术,可以初步发现了解原发性肝癌外肝动脉供血血管及肝动脉血供情况,对指导肝癌介入治疗插管、患者选择及治疗效果的预测将有一定的临床价值。
Objective To investigate the diagnosis and clinical value of hepatic arterial phase angiography with MSCT in the diagnosis of primary liver cancer with extrahepatic arterial blood supply. Methods Three-phase MSCT dynamic scanning of primary liver cancer was performed. In addition to conventional axial and MPR observations, the three-dimensional revascularization of the hepatic artery system was performed. The reconstructed methods included maximum intensity projection (MIP), volume reconstruction (VR) Cover image (SSD) and the like. Results A total of 39 extrahepatic donor arteries were found in 35 patients. The types of extrahepatic arteries originated from the internal thoracic artery (5.1%), the right intercostal artery (7.7%), the left gastric artery (12.8%), the right subphrenic artery (38.5%), the omental artery ), Right adrenal or renal capsule artery (10.3%). Conclusion Three-dimensional reconstruction of MSCT hepatic artery system can initially find out the situation of hepatic arterial blood supply and hepatic artery blood supply in patients with primary hepatic carcinoma. It will be helpful to predict the selection and treatment effect of intubation, value.
