目的:对急性呼吸窘迫综合征(ARDS)患者运用早期机械通气治疗,探讨与分析其对血流动力学和血氧饱和度的影响。方法:回顾性分析我院于2010年10月到2013年10月收治的58例ARDS患者的临床资料,通过医院伦理学委员会依照随机数字表的方式对其中29例患者出现ARDS24h内实施机械通气(早期通气组),另29患者在出现ARDS24h后行机械通气(晚期通气组),对两组的血流动力学和血氧饱和度情况进行观察与对比。结果:HR、MPAP、PAWP上,早期通气组要显著低于晚期通气组(P<0.05);在PaO2、PaO2/FiO2和PvO2上,早期通气组要高于晚期通气组(P<0.05)。结论:针对ARDS患者,早期实施机械通气治疗可使患者的血氧饱和度得到显著改善,使肺动脉高压得到有效纠正,并使患者的血流动力学趋于稳定,值得推广。
OBJECTIVE: To investigate the effects of early mechanical ventilation on patients with acute respiratory distress syndrome (ARDS) and on hemodynamics and oxygen saturation. Methods: The clinical data of 58 ARDS patients admitted from October 2010 to October 2013 in our hospital were analyzed retrospectively. Twenty-nine of the patients underwent ARDS within 24 hours after admission through the Hospital Ethics Committee according to the random number table Early ventilatory group). The other 29 patients underwent mechanical ventilation 24 h after ARDS (late ventilatory group). The hemodynamics and oxygen saturation of the two groups were observed and compared. Results: Compared with the late ventilation group (P <0.05), the early ventilation group on HR, MPAP and PAWP was significantly lower than that of the late ventilation group on PaO2, PaO2 / FiO2 and PvO2 (P <0.05). Conclusion: For patients with ARDS, the early implementation of mechanical ventilation can significantly improve the patient’s oxygen saturation, effective correction of pulmonary hypertension and make the patient’s hemodynamics stable and worth promoting.
