目的 观察关节腔注射光动力疗法对兔膝骨性关节炎血清骨钙素(osteocalcin,OC)与Ⅱ型胶原羧基端端肽(Carboxy-TerminalTelepeptides of TypeⅡCollagen,CTX-Ⅱ)水平影响,探讨光动力疗法(Photodynamic therapy,PDT)对骨性关节炎的治疗效果.方法 将25只日本大耳白兔随机分为5组,分别为空白对照组、模型对照组及3个光动力治疗组(PDT1组、PDT2组及PDT3组),每组各5只.除空白对照组外均以改良Hulth法建立骨性关节炎模型;PDT1组、PDT2组及PDT3组分别施以2、4和6 mg光敏剂,采用波长650 nm激光关节外照射,功率密度130 mW/cm2,能量密度128 J/cm2.5组处理后,采用HE染色,观察软骨组织形态的变化,并用酶联免疫吸附法(Enzyme-Linked Immuno Sorbent Assay,ELISA)检测血清中OC与CTX-Ⅱ水平的变化,对实验结果进行统计分析.结果 (1)光镜下三个PDT组的关节软骨组织结构及软骨细胞形态有不同程度的恢复,以PDT3组治疗效果最优.(2)各组血清CTX-Ⅱ均值由高到低依次为:模型组>PDT1组>PDT2组>PDT3组>空白对照组,各组间比较差异具有统计学意义(P<0.05).(3)各组血清OC均值由高到低依次为:空白组>PDT2组>PDT1组>PDT3组>模型组;除PDT1组和PDT2组(P>0.05);PDT2组和空白组(P>0.05),组间的比较差异无统计学意义外,其余各组间比较的差异均具有统计学意义(P<0.05).结论 关节腔注射光敏剂联合关节外辐照的光动力疗法可以减缓软骨降解,降低软骨损伤程度,延缓骨性关节炎的病理进程.“,”Objective To investigate the effects of photodynamic therapy on osteoarthritis by observing the serum levels of CTX-Ⅱ[and OC in rabbits given both external photodynamic therapy (PDT) and intra-articular injection.Methods An experimental study was performed on 25 big-eared Japanese rabbits.All these rabbits were randomly assigned to five groups:the blank control group,model control group and three photodynamic therapy groups (PDT1 group,PDT2 group and PDT3 group).Except for the blank control group,models were established in other groups with modified Hulth method.The rabbits in the PDT1 group,PDT2 group and PDT3 group were given injection of 2 mg,4 mg and 6mg photosensitizer respectively and the photodynamic therapy 4 times.HE staining was used to observe the expression change of cartilage histomorphology,and enzyme-linked immunoabsorbentassay (ELISA) was applied to detect the serum levels of CTX-Ⅱ and OC.A statistical analysis of the experiment results was then made.Results (1) HE staining:The cartilage histomorphology in the three PDT groups were significantly better than that of the model control group.Among them,the result of the PDT3 group was optimal.(2) As to the average level of serum CTX-Ⅱ,the sequence from the highest to the lowest was the model group,PDT1 group,PDT2 group,PDT3 group and blank group (P < 0.05).(3) For average level of serum OC,the sequence from the highest to the lowest was the blank group,PDT2 group,PDT1 group,PDT3 group and model group.Except for the comparison between the PDT1 group and PDT2 group (P > 0.05) and the comparison between the PDT2 group and blank group (P > 0.05),the differences between other groups were statistically significant (P < 0.05).Conclusions Joint application of intra-articular injection and external photodynamic therapy (PDT) will slow down degradation of cartilage,reduce severity of cartilage injury and delay pathological process of osteoarthritis.
