目的:观察厚朴排气合剂对剖宫产术后患者胃肠功能恢复的作用及对血清胃泌素(GAS)、胃动素(MTL)和血管活性肠肽(VIP)的影响。方法:90例剖宫产妇随机分为对照组和观察组各45例。对照组给予西医常规对症处理。观察组采用厚朴排气合剂口服。2组疗程为3天。记录术后肠鸣音恢复时间;记录术后首次排气、排便时间;分别于术后第1、2、3天进行记录主要症状、体征评分;检测术后第1、3天患者血GAS、MTL和VIP水平。结果:观察组肠鸣音恢复时间、首次排气和排便时间均短于对照组,差异均有非常显著性意义(P<0.01)。术后第2天对照组主要症状、体征评分较术后第1天有所下降,但差异无显著性意义(P>0.05),术后第3天,对照组主要症状、体征评分较术后第1天下降,差异有显著性意义(P<0.05);观察组术后第2、3天主要症状、体征评分均较术后第1天明显下降,差异均有显著性意义(P<0.05),并低于同期对照组(P<0.05)。术后第3天,对照组血清GAS、MTL和VIP水平变化不明显;观察组术后第3天GAS、MTL和VIP水平均较术后第1天明显变化,差异均有非常显著性意义(P<0.01),与对照组各项指标比较,差异均有非常显著性意义(P<0.01)。结论:厚朴排气合剂能促进剖宫产术后产妇胃肠功能的恢复,改善症状和体征,其作用机制可能与升高患者血清GAS和MTL水平,抑制VIP水平有关。
OBJECTIVE: To observe the effects of Houpu Exhalation Mixture on the recovery of gastrointestinal function in patients after cesarean section and on the levels of serum gastrin (GAS), motilin (MTL) and vasoactive intestinal peptide (VIP). Methods: 90 cases of cesarean section were randomly divided into control group and observation group of 45 cases. The control group was given conventional Western symptomatic treatment. The observation group was treated with Houpi Exhalation Mixture orally. 2 groups of treatment for 3 days. The postoperative bowel sounds recovery time was recorded. The first postoperative exhaust and defecation time were recorded. The main symptoms and signs were recorded on the 1st, 2nd and 3rd postoperative days respectively. The levels of GAS, MTL and VIP levels. Results: The recovery time of bowel sounds, the time of the first exhaust and defecation in the observation group were shorter than those in the control group, with significant differences (P <0.01). On the second day after operation, the main symptoms and signs of the control group decreased compared with the first day after operation, but the difference was not statistically significant (P> 0.05). On the third day after operation, the main symptoms and signs of the control group were significantly (P <0.05). The scores of major symptoms and signs on the 2nd and 3rd postoperative days in the observation group were significantly lower than those on the 1st postoperative day (P <0.05), and the differences were statistically significant ) And lower than the control group (P <0.05). The levels of serum GAS, MTL and VIP in the control group did not change significantly on the third day after operation. The levels of GAS, MTL and VIP on the third day after the operation in the observation group were significantly changed compared with those on the first day after operation, with significant differences (P < P <0.01). Compared with the control group, the differences were significant (P <0.01). CONCLUSION: HOPU mixture can promote the recovery of gastrointestinal function and improve the symptoms and signs of postpartum women after cesarean section. The mechanism may be related to increasing serum GAS and MTL levels and inhibiting VIP level.
