对11例原发性肺动脉高压患者进行回顾性分析,其中8例同时行右心导管检查。结果表明,劳力性呼吸困难、晕厥和水肿是主要临床表现;超声心动图检查示右房、右室及主肺动脉扩大,应用连续波多普勒估测肺动脉压是一种比较可靠的无创性技术(r=0.93);常规应用血管扩张剂和抗凝剂疗效差。结合文献认为肺血管内皮细胞损害是其发病基础。
11 patients with primary pulmonary hypertension were retrospectively analyzed, of which 8 cases underwent right heart catheterization at the same time. The results showed that dyspnea of effort, syncope and edema were the main clinical manifestations. Echocardiography showed that the right atrium, right ventricle and the main pulmonary artery were enlarged. Using continuous wave Doppler to estimate pulmonary arterial pressure was a reliable noninvasive technique (r = 0.93); conventional vasodilators and anticoagulants poor efficacy. Combined with the literature that pulmonary vascular endothelial cell damage is the basis of its pathogenesis.
