目的探讨血清中基质金属蛋白酶9(MMP9)和血管内皮生长因子(VEGF)对胰腺癌的诊断作用及二者的相关性。方法收集45例胰腺癌患者和76例健康体检者血清标本,ELISA法检测其中MMP9和VEGF浓度,并做统计学分析。结果胰腺癌组血清中MMP9和VEGF含量均明显高于健康体检者水平,差异具有统计学意义;分化程度低者二者水平高于分化程度中高者,但无明显差异;I、II期患者MMP9和VEGF水平均高于III、IV期患者,具有统计学意义;胰腺癌血清MMP9和VEGF水平与淋巴结转移与否相关,与性别、年龄及病理分型无明显相关性;胰腺癌患者VEGF和MMP9含量呈正相关。结论胰腺癌患者血清MMP9和VEGF可作为其诊断指标,与淋巴结转移和临床分期具有相关性;胰腺癌侵袭及转移与其新生血管形成有着重要联系。其机制可能是VEGF通过诱导内皮细胞产生MMPs或通过调控其抑制物的平衡,激活基质降解的一系列反应,使肿瘤细胞穿透性增强,更利于肿瘤的浸润、转移。
Objective To investigate the diagnostic value of serum matrix metalloproteinase 9 (MMP9) and vascular endothelial growth factor (VEGF) in pancreatic carcinoma and their relationship. Methods Serum samples of 45 patients with pancreatic cancer and 76 healthy controls were collected. The concentrations of MMP9 and VEGF were detected by ELISA and analyzed statistically. Results The serum levels of MMP9 and VEGF in pancreatic cancer group were significantly higher than those in healthy subjects, the difference was statistically significant; the level of differentiation was lower than the higher level of differentiation, but there was no significant difference; And VEGF levels were higher than those in stage III and IV patients with statistical significance. The levels of serum MMP9 and VEGF in pancreatic cancer were not correlated with lymph node metastasis, but with gender, age and pathological type. The expressions of VEGF and MMP9 Content was positively correlated. Conclusion The serum levels of MMP9 and VEGF in patients with pancreatic cancer can be used as a diagnostic indicator of lymph node metastasis and clinical stage. The invasion and metastasis of pancreatic cancer are closely related to their neovascularization. The mechanism may be VEGF through the induction of endothelial cells to produce MMPs or by regulating the balance of their inhibitors, activation of matrix degradation of a series of reactions, the tumor cell penetration increased, more conducive to tumor invasion and metastasis.
