目的探讨雷公藤多苷联合坎地沙坦治疗糖尿病肾病的临床疗效。方法选择2014年1月至2015年5月治疗的60例糖尿病肾病患者的临床资料进行回顾性分析。对照组30例采用坎地沙坦治疗,研究组30例在坎地沙坦治疗基础上联合雷公藤多苷治疗。比较两组的临床疗效。结果治疗后,两组24 h尿蛋白量较治疗前均显著下降(P<0.01);研究组24 h尿蛋白量显著低于对照组(P<0.01)。治疗后,两组白蛋白含量较治疗前显著升高(P<0.01);研究组白蛋白含量显著高于对照组(P<0.01)。两组治疗前后肌酐、肌酐消除率水平变化不明显,差异未见统计学意义(P>0.05)。两组治疗前后肝功能丙氨酸氨基转移酶、碱性磷酸酶水平变化不明显,差异未见统计学意义(P>0.05);白细胞水平治疗前后差异也未见统计学意义(P>0.05)。研究组有1例患者治疗期间有轻度肝功能指标升高,采用保肝药物治疗恢复正常,无严重感染病例,白细胞计数无低于3×109/L的病例。结论雷公藤多苷联合坎地沙坦治疗糖尿病肾病能够显著减少24 h尿蛋白量,升高血清白蛋白含量,并且不良反应少,值得临床推广。 Objective To investigate the clinical efficacy of tripterygium glycosides combined with candesartan in the treatment of diabetic nephropathy. Methods The clinical data of 60 patients with diabetic nephropathy treated from January 2014 to May 2015 were retrospectively analyzed. Thirty patients in the control group were treated with candesartan, and 30 in the study group were treated with candesartan in combination with candesartan. The clinical efficacy of the two groups was compared. Results After 24 hours of treatment, urinary protein levels of 24 h in both groups decreased significantly (P <0.01). Urine protein in 24 hours of study group was significantly lower than that in control group (P <0.01). After treatment, the content of albumin in the two groups was significantly higher than that before treatment (P <0.01). The content of albumin in the study group was significantly higher than that in the control group (P <0.01). Before and after treatment, creatinine and creatinine elimination rate did not change significantly, the difference was not statistically significant (P> 0.05). There was no significant difference between the two groups in liver function before and after alanine aminotransferase, alkaline phosphatase, the difference was not statistically significant (P> 0.05); There was no significant difference in the level of leukocyte before and after treatment (P> 0.05) . One patient in the study group had mild liver function indexes during treatment, returned to normal without hepatotoxic drugs, and had no leukocyte count of less than 3 × 109 / L. Conclusion Tripterygium glycosides combined with candesartan in the treatment of diabetic nephropathy can significantly reduce 24 h urinary protein, increase serum albumin, and less adverse reactions, is worth clinical promotion.
