目的研究糖尿病手术(胃转流手术Gastric bypass,GBP)对II型糖尿病大鼠空腹血糖和餐后血糖调控的效果。方法计算机检索PubMed、中文期刊全文数据库、维普中文科技期刊数据库、万方数据库收录的1979年1月~2010年12月发表的关于GBP降低Ⅱ型糖尿病大鼠血糖相关的随机对照研究,并辅以手工检索,对照纳入标准将筛选文献,对符合纳入标准的研究的质量进行评价,用RevMan5.1软件进行Meta分析。结果共收集到相关文献37篇,其中符合纳入标准的有14篇,总的有效量合并RR值为0.79(95%CI:0.66~0.94),其中符合术后4周空腹血糖研究纳入标准的有9篇,其效应尺度是同质的,空腹血糖浓度合并RR值0.81(95%CI:0.76~0.86),其中符合餐后血糖研究纳入标准的有10篇,其效应尺度是同质的,术后4周OGTT合并RR值0.71(95%CI:0.67~0.75),发表偏倚估计t=0.332,P=0.749﹥0.05。结论 GBP手术对降低II型糖尿病大鼠空腹血糖(FBG)和餐后血糖(OGTT试验)有显著的效果,并且具有一定时间的长效性。
Objective To investigate the effects of diabetic surgery (Gastric bypass, GBP) on fasting blood glucose and postprandial blood glucose in type 2 diabetic rats. Methods PubMed, Chinese Journal Full-text Database, VIP Chinese Sci-tech Periodical Database, Wanfang database were searched from January 1979 to December 2010, and randomized controlled studies about the effect of GBP on reducing blood glucose in type 2 diabetic rats were performed. Manually searched, screened against controlled inclusion criteria, evaluated for quality of studies that met inclusion criteria, and Meta-analyzed using RevMan 5.1 software. Results A total of 37 articles were collected, of which 14 met the inclusion criteria. The combined effective rate (RR) was 0.79 (95% CI: 0.66-0.94), which was consistent with the 4-week fasting glucose test 9, the effect scale is homogeneous, the fasting blood glucose concentration with RR value of 0.81 (95% CI: 0.76 ~ 0.86), which meet the postprandial blood glucose research into the standard of 10, the effect scale is homogeneous, surgery OGTT combined with RR of 0.71 (95% CI: 0.67-0.75) in the last 4 weeks and publication bias t = 0.332, P = 0.749> 0.05. Conclusion The GBP operation has a significant effect on reducing fasting blood glucose (FBG) and postprandial blood glucose (OGTT) in type 2 diabetic rats and has a long-term effect.
