目的 :评价小剂量抑肽酶在小儿体外循环 (CPB)手术中的作用。方法 :将 86例先天性心脏病患儿分为抑肽酶用药组 (4 0例 )和对照组 (4 6例 ) ,观察两组患儿术后 2 4h纵隔心包引流量 ,并从两组中各抽取 1 8例监测凝血和纤溶各项指标。结果 :与对照组比较用药组 CPB术后 Plat、PT、FIB无显著影响 ,APTT缩短 ,PL G、AT- 活性明显下降 ,α2 -AP活性明显提高。术后 3h用药组APTT仍低于对照组 ,α2 -AP活性则高于对照组。术后 2 4h用药组心包纵隔引流血量较对照组显著减少 (P≤ 0 .0 1 )。结论 :在小儿体外循环手术中应用小剂量抑肽酶能减少术后出血
Objective: To evaluate the role of low-dose aprotinin in pediatric cardiopulmonary bypass (CPB) surgery. Methods: 86 children with congenital heart disease were divided into aprotinin group (40 cases) and control group (46 cases). The mediastinal pericardial drainage was observed at 24 hours after operation in both groups. In the extraction of 18 cases of coagulation and fibrinolysis monitoring of various indicators. Results: Compared with the control group, there was no significant effect of Plat, PT and FIB after CPB treatment. The APTT was shortened, the activity of PL G and AT- was significantly decreased and the activity of α2-AP was significantly increased. APTT was still lower than the control group 3h after operation, α2-AP activity was higher than the control group. At 24 hours after operation, the drainage volume of pericardial drainage in the medication group was significantly lower than that in the control group (P≤0.01). Conclusion: Applying a small dose of aprotinin in pediatric cardiopulmonary bypass surgery can reduce postoperative bleeding
