探讨螺旋 CT三维及四维重建显示关节内骨折的价值及其临床意义.方法使用PICKER 6000螺旋 CT机对 37例患者的损伤关节进行扫描,在 VOXELQ工作站上进行三维表面遮盖法重建;以容积重建技术实现四维重建;使用关节解体处理技术以充分展示损伤的关节面;在Z轴及X轴上旋转重建图像观察损伤细节.结果 三维及四维重建图像清晰地显示了所有病例的骨折细节;结合关节解体技术,三维重建逼真地显示了损伤的立体表面形态,而对显示骨质内部损伤以及骨折块之间、关节各部分之间的相互位置关系,四维重建则明显优于三维重建.结论 将螺旋 CT三维、四维重建技术、关节解体技术与图像旋转技术相结合,可以显示任何一种复杂关节内骨折的全部细节,达到直视的效果.三维及四维重建还有助于对手术入路及内固定方法的设计,有助于术前手术器械的准备以及帮助理解骨折的复杂性.“,”Objective to discuss the value of spiral computed tomography (SCT) in revealing de tails of intra-articlar fractures with 3-and 4-dimensional )3-d and 4-d_ reconstruction and related clinical significance. Methods Thirty-seven cases of intra-articular fracture were scanned with Picker 6000 spiral CT scanner, and the data obtained were treated in Voxel Q work station for 3-d reconstruction with shaded surface display technique and 40d (volumetric) reconstruction with volumetric rendering technique. Joint-separation technique was used to show detail of joint surface involved. The reconstructed image was ro tated round X and Z axes for better appreciation of details of the fractures. Results SCT revealed every detail of the fractures in 3-d and 4d images of all patients. 3-d images cleary drew the stereoscopic injured surfaces with the help of joint-separation technique. For appreciation of fracture in the bone and stereoscopic relationship between bone pieces of different parts of the joint ,4-d image was superior to 3-d image. Con clusion With the combination of 3-d, 4-d, joint-separation and image-rotation technique, every detail of any complex fracture can be perceived directly and distinctly. 3-d and 4-d reconstruction is helpful in designing operative approach and internal fixation method, preparing surgical instruments and explaining the complexity of the fractures.
