高渗盐水治疗对失血性休克鼠网状内皮细胞功能和易感染性的影响

目的 探讨高渗盐水治疗对失血性休克鼠网状内皮细胞功能和易感染性的影响。方法 心脏穿刺抽取小鼠总血量的 4 0 % ,制成失血性休克动物模型 2h后 ,分别经尾静脉输注贮血和生理盐水或 7.5 %NaCl治疗 ,观察两种方法治疗后再行盲肠结扎穿刺 (CLP)鼠的存活率。以碳粒廓清实验和肝、脾巨噬细胞摄取碳粒量检测网状内皮细胞的吞噬功能。同时经体外实验观察不同渗透压对腹腔巨噬细胞吞噬中性红功能的影响。结果 高渗盐水治疗后 2 4h行CLP鼠的存活率为 70 % ,而生理盐水治疗组则全部死亡。休克后 3h高渗盐水治疗组网状内皮细胞对碳粒廓清指数α以及肝脏巨噬细胞摄取碳粒量 (5 .6 1± 0 .4 2 ,0 .5 9± 0 .19)均显著高于生理盐水治疗组(4 .15± 0 .6 2 ,0 .4 2± 0 .16 ) ,P<0 .0 1。体外实验 ,在 4 0mmol/L内额外升高培养液的渗透压 ,对小鼠腹腔巨噬细胞吞噬中性红的量无显著影响。结论 高渗盐水治疗可以间接改善失血性休克鼠网状内皮细胞吞噬功能并降低感染的易感性。 Objective To investigate the effects of hypertonic saline on reticuloendothelial function and susceptibility to hemorrhagic shock in rats. Methods Cardiac puncture was used to extract 40% of the total amount of blood in mice. After 2 hours of hemorrhagic shock, the animals were treated by intravenous infusion of blood and normal saline or 7.5% NaCl respectively. The two methods were followed by cecum Survival of ligation and puncture (CLP) mice. The carbon clearance test and the amount of carbon uptake by liver and spleen macrophages were used to detect the phagocytic function of reticuloendothelial cells. At the same time, we observed the effect of different osmotic pressure on phagocytic function of neutral macrophages in vitro. Results The survival rate of CLP rats was 70% at 24 h after hypertonic saline treatment, while all the saline treated rats died. 3h after shock, reticuloendotheliosis of reticuloendotheliosis-treated reticuloendotheliocytes was significantly higher than that of carbon clearance index (α) and carbon uptake by liver macrophages (5.61 ± 0.42, 0.59 ± 0.19) In the saline treatment group (4.15 ± 0.62, 0.42 ± 0.16), P <0.01. In vitro experiments, an additional increase in culture medium osmolality at 40 mmol / L had no significant effect on the amount of phagocytosis of neutral red in mouse peritoneal macrophages. Conclusion Hypertonic saline treatment can indirectly improve the phagocytic function of reticular endothelial cells in hemorrhagic shock mice and reduce the susceptibility to infection.
