目的探讨煤矿工人血红蛋白水平与代谢综合征(MS)及其组分(血压、血脂、血糖、肥胖)的相关性。方法以河北省某矿业集团在岗职业健康检查人群为研究对象。按血红蛋白(Hb)水平四分位数分为4组,以SPSS 17.0统计软件分析其Hb水平与收缩压(SBP)、舒张压(DBP)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FPG)、体质指数(BMI)之间的相关性。结果煤矿工人MS患病率17.73%,MS组Hb[(150.26±11.12)g/L],显著高于非MS组Hb[(144.64±12.35)g/L],差异有统计学意义(P<0.01),不同Hb的水平与年龄、SBP、DBP、TC、TG、HDL-C、FPG、BMI比较,差异均有统计学意义(均P<0.01),Hb最低四分位组到最高四分位组,随着Hb水平的上升,SBP、DBP、TC、TG、FPG、BMI各指标水平升高,HDL-C水平降低。通过Pearson分析,研究对象的Hb与TG、TC、FPG、SBP、DBP、BMI等均呈显著正相关(r值分别为0.209、0.165、0.144、0.237、0.218、0.127,均P<0.01),与HDL-C呈负相关(r值为-0.175,P<0.01)。多因素逐步回归分析后显示,Hb的升高与MS发病风险的增加呈正相关(β=0.123、0.185、0.122、0.130、-0.204、0.104)。结论煤矿工人Hb与MS及血压、血脂、血糖、BMI存在相关性,可为该人群MS的早期诊断提供参考。
Objective To explore the correlation between hemoglobin level of coal miners and metabolic syndrome (MS) and its components (blood pressure, blood lipid, blood sugar, obesity). Methods Taking the mining occupational health examination population of a mining group in Hebei Province as the research object. The levels of hemoglobin (Hb) were divided into four groups. SPSS 17.0 software was used to analyze the relationship between Hb level and SBP, DBP, TG, TC, HDL-C, fasting blood glucose (FPG) and body mass index (BMI). Results The prevalence of MS was 17.73% in coal miners and Hb [(150.26 ± 11.12) g / L in MS group was significantly higher than that in non-MS group (144.64 ± 12.35 g / L, P < 0.01). The differences of Hb level with age, SBP, DBP, TC, TG, HDL-C, FPG and BMI were all statistically significant (all P <0.01) As the level of Hb increased, the level of SBP, DBP, TC, TG, FPG and BMI increased and the level of HDL-C decreased. Pearson analysis showed that there was a significant positive correlation between Hb and TG, TC, FPG, SBP, DBP and BMI (r = 0.209, 0.165,0.144,0.237,0.218,0.127, all P <0.01) HDL-C was negatively correlated (r value -0.175, P <0.01). Multi-factor regression analysis showed that elevated Hb was positively correlated with the increased risk of MS (β = 0.123,0.185,0.122,0.130,0.204,0.104). Conclusion There is a correlation between Hb, MS and blood pressure, blood lipid, blood sugar and BMI in coal miners, which may provide a reference for the early diagnosis of MS in this population.
