目的 了解人工耳蜗植入前配戴助听器对人工耳蜗植入后康复的作用。方法 选取人工耳蜗植入后进行康复训练的 5 8例患者 ,其中男 36例 ,女 2 2例 ,年龄为 2 .5～ 3岁。按照术前配戴助听器的时间不同 ,分为A、B、C 3组 ,A组为 12例没有配戴过助听器的患者 ,B组为 15例配戴了 2～ 4个月助听器的患者 ,C组为 31例配戴了 10～ 13个月助听器的患者 ,分别测试植入并开机训练后 1周、1个月、3个月的补偿听阈、Ling’s 6音、听音辨图等项目。结果 开机后 1周和 1个月时 3组的补偿听阈之间差异均无显著性 (P >0 .0 5 ) ,B、C组在开机 3个月时的补偿听阈要比A组好 (P <0 .0 5 ) ;开机 1周和开机 1个月时B、C组Ling’s 6音的识别率要比A组明显高 (P <0 .0 1) ,且C组的识别率也要比B组明显高 (P <0 .0 1) ,而在开机 3个月时3组的识别率无明显差异 ;A、B、C 3组在同期的听音辨图的能力依次增高 (P <0 .0 1)。结论人工耳蜗术前配戴助听器比未配戴助听器的效果好 ,术前配戴助听器 10～ 13个月的比配戴了 2～ 4个月的效果好
Objective To understand the effect of hearing aid worn before cochlear implantation on post-cochlear implant rehabilitation. Methods Fifty-eight patients with rehabilitation training after cochlear implantation were enrolled, including 36 males and 22 females with a mean age of 2.5-3 years. According to the preoperative hearing aid wearing time is divided into A, B, C 3, A group of 12 patients without wearing hearing aids, B group of 15 patients wearing a hearing aid 2-4 months, In group C, 31 patients with hearing aids who had been wearing for 10-13 months were tested for compensatory hearing thresholds, Ling’s 6-tone, audiogram and other items at 1 week, 1 month and 3 months after implantation and training. Results There was no significant difference in hearing threshold between the three groups at 1 week and 1 month after starting up (P> 0.05), while the compensatory hearing threshold at 3 months after startup in group B and C was better than that in group A P <0.05). The recognition rate of Ling’s 6 tone in B and C groups was significantly higher than that in group A (P <0. 01) at 1 week after starting up and 1 month after starting up, and the recognition rate in group C (P <0.01), but there was no significant difference in the recognition rate among the three groups at 3 months after starting up (P <0 .0 1). Conclusion The preoperative cochlear hearing aids are better than those without a hearing aid. The preoperative hearing aids are better than wearing hearing aids for 10 to 13 months than those who wearing them for 2 to 4 months
