病例介绍患者男性,50岁。因左下眼睑黑色肿物4年入院。主诉4年前因工作不慎左下眼睑被高热铁屑烧伤,愈合后留有直径0.4cm大小瘢痕,之后出现黑色素沉着。近期逐渐扩大,无疼痛,未作任何治疗。眼科检查:左上眼睑鼻侧见一直径1cm大小肿物,周边隆起,边界清楚,表面不光滑,灰黑色,质软,无溃烂及压痛,全身浅表淋巴结不肿大。临床诊断:黑色素瘤。病理检查:带有梭形皮肤的扁圆形包块一个,1.2cm×1cm×0.5cm大小,表面黑褐色,不光滑,无溃烂。切面灰黑色,肿瘤无包膜。镜下见肿瘤组织位于真皮浅层。瘤细胞多呈实性巢状或条索状排列,其边缘细胞为梭形,呈 Case presentation Male patient, 50 years old. He was admitted to hospital for 4 years because of a black mass in the left lower eyelid. The chief complaint was 4 years ago due to work inadvertently left lower eyelid burned by high heat iron, after healing, left a diameter of 0.4cm scar, after melanin pigmentation. Recently gradually expanded, no pain, no treatment. Eye examination: The left upper eyelid saw a 1cm-diameter mass on the nasal side. The periphery was uplifted with clear boundaries. The surface was not smooth, grayish black, soft, no ulceration and tenderness, and the superficial lymph nodes were not swollen. Clinical diagnosis: melanoma. Pathological examination: a flat circular mass with a spindle-shaped skin, 1.2cm × 1cm × 0.5cm size, dark brown surface, not smooth, no ulceration. The cut surface is grayish black and the tumor has no capsule. Microscopically, the tumor tissue was located in the superficial dermis. Tumor cells are mostly solid nests or cords, and their limbal cells are spindle-shaped.
