目的探讨肺部孤立性结节或肿块MSCT灌注成像与微血管密度(microvessel density,MVD)的相关性。方法对33例肺部孤立性结节或肿块患者于经皮肺穿刺活检或手术前行16层螺旋CT灌注扫描,病理标本常规HE染色及单克隆抗CD34抗体标记血管,将灌注参数值和微血管密度进行对照分析。结果周围型肺癌(恶性组)20例,活动性炎性结节或肿块(炎性组)5例,其他良性结节或肿块(良性组)8例。恶性和炎性组的MVD明显大于良性组(P<0.01),而恶性与炎性两组间MVD没有统计学差异(P>0.05)。除MTT外,BF、BV和PS与MVD之间均呈线性正相关(P<0.01),其中以BV值相关系数最高。结论肺结节或肿块的BF、BV和PS值与MVD之间呈正相关,MSCT灌注成像为在活体无创性评价肺肿瘤的MVD情况提供了一条新的途径。
Objective To investigate the correlation between MSCT perfusion imaging and microvessel density (MVD) in solitary pulmonary nodules or lumps. Methods Thirty-three patients with solitary pulmonary nodules or lumps underwent 16-slice spiral CT perfusion scanning before percutaneous lung biopsy or surgery. Pathological specimens were stained with HE and labeled with anti-CD34 monoclonal antibody. The perfusion parameters and microvessel Density control analysis. Results There were 20 cases of peripheral lung cancer (malignant group), 5 cases of active inflammatory nodules or masses (inflammatory group) and 8 cases of other benign nodules or masses (benign group). The MVD in malignant and inflammatory groups was significantly higher than that in benign groups (P <0.01), but there was no significant difference between malignant and inflammatory groups (P> 0.05). In addition to MTT, there was a linear positive correlation between BF, BV, PS and MVD (P <0.01), with the highest BV value. Conclusions The BF, BV and PS values of pulmonary nodules or lumps are positively correlated with MVD. MSCT perfusion imaging provides a new way to noninvasively evaluate MVD in lung tumors.
