目的研究环氧化酶-2(COX-2)与芳香化酶(P450arom)在单纯与复杂增生子宫内膜、非典型增生内膜及子宫内膜癌组织中的表达,并分析两者表达的相关性和意义。方法选取94例手术及刮宫组织标本,其中正常子宫内膜10例、单纯与复杂增生子宫内膜29例、非典型增生子宫内膜17例、子宫内膜癌38例,运用免疫组织化学Envision法检测COX-2与P450arom蛋白的表达。结果COX-2蛋白在单纯与复杂增生子宫内膜、非典型增生子宫内膜和子宫内膜癌中的表达均高于正常子宫内膜,差异均有统计学意义(χ2=5.283~14.928,P<0.05);P450arom蛋白在子宫内膜癌中的表达高于正常子宫内膜,差异有统计学意义(2χ=9.023,P<0.01);在单纯与复杂增生子宫内膜、非典型增生子宫内膜中的表达与正常子宫内膜比较,差异均无统计学意义(P>0.05)。而在子宫内膜癌中,两者的表达与组织学分级及手术病理分期均无关(P>0.05)。COX-2和P450arom蛋白在子宫内膜癌及癌前病变中的表达密切相关(r=0.598,P<0.01)。结论COX-2与P450arom蛋白在子宫内膜癌及癌前病变中呈高表达,且两者的表达呈正相关,提示两者可能存在协同作用,共同促进子宫内膜癌的发生和发展。
Objective To study the expression of cyclooxygenase-2 (COX-2) and aromatase (P450arom) in simple and complex proliferative endometrium, atypical hyperplastic endometrium and endometrial carcinoma, Relevance and meaning. Methods Ninety-four cases of surgical and curettage specimens were selected, including 10 cases of normal endometrium, 29 cases of simple and complex hyperplasia endometrium, 17 cases of atypical hyperplasia endometrium and 38 cases of endometrial carcinoma. Immunohistochemistry Envision method The expression of COX-2 and P450arom protein was detected. Results The expression of COX-2 protein in simple and complex hyperplasia endometrium, atypical hyperplasia endometrium and endometrial carcinoma were significantly higher than those in normal endometrium (χ2 = 5.283-14.928, P <0.05). The expression of P450arom protein in endometrial carcinoma was significantly higher than that in normal endometrium (2χ = 9.023, P <0.01). In simple and complex hyperplasia of endometrium, atypical hyperplasia uterus Membrane expression in normal endometrium, the difference was not statistically significant (P> 0.05). In endometrial carcinoma, the expression of both was not associated with histological grade and pathological stage (P> 0.05). The expression of COX-2 and P450arom protein in endometrial carcinoma and precancerous lesions were closely related (r = 0.598, P <0.01). Conclusions COX-2 and P450arom proteins are highly expressed in endometrial carcinoma and precancerous lesions, and the expression of COX-2 and P450arom is positively correlated, suggesting that there may be a synergistic effect between COX-2 and P450arom to promote the occurrence and development of endometrial carcinoma.
