房室传导的空隙(Gap)现象,是指在心动周期的某时限内到达的房性冲动不能传入心室,而较早或较晚的房性冲动却能传入心室,这一时限区域称为空隙期。这一现象是房室传导分层现象的表现,可发生在前向或逆向的房室传导过程中。用心腔内多电极导管检测技术可同时描记到心房、希氏束、心室电活动波,用心房程序调搏可以得到稳定的原始周期和预计出现的房性早搏,从而能了解冲动下传的情况,为研究空隙现象的机理提供了精确而可靠的方法。本文则以常规的体表心电图作为分析基础,对3例房室传导的空隙现象作一分析和讨论。
