目的探讨腹腔镜联合术前新辅助放化疗用于低位直肠癌保肛术对患者血清血管内皮生长因子(VEGF)、癌胚抗原(CEA)、人瘦素(LEP)水平的影响。方法将行低位直肠癌保肛术的低位直肠癌患者182例,按照手术方式不同分为观察组122例和对照组60例,观察组使用腹腔镜联合术前新辅助放化疗的低位直肠癌保肛术治疗,对照组使用开放手术联合术前新辅助化疗。分别检测2组治疗前后血清VEGF、CEA、LEP水平。结果治疗后2组患者血清VEGF、CEA水平均低于治疗前,LEP水平高于治疗前,且观察组变化幅度大于对照组,差异均有统计学意义(P<0.05)。结论腹腔镜联合术前新辅助放化疗用于低位直肠癌保肛术能有效地降低患者血清VEGF、CEA水平和提高血清LEP水平。
Objective To investigate the effect of preoperative neoadjuvant chemoradiotherapy combined with laparoscopy in the treatment of low rectal cancer with anal sphincter on the serum levels of vascular endothelial growth factor (VEGF), carcinoembryonic antigen (CEA) and human leptin (LEP). Methods A total of 182 patients with low rectal cancer undergoing anal sphincter preservation surgery for low rectal cancer were divided into observation group (122 cases) and control group (60 cases) according to different surgical methods. The observation group received laparoscopy combined with preoperative neoadjuvant radiotherapy and chemotherapy for rectal cancer Anal surgery, the control group using open surgery combined with preoperative neoadjuvant chemotherapy. The serum levels of VEGF, CEA and LEP were measured before and after treatment in two groups. Results After treatment, the levels of serum VEGF and CEA in both groups were lower than those before treatment. The levels of LEP were higher than those before treatment, and the changes in the observation group were greater than those in the control group. The differences were statistically significant (P <0.05). Conclusions Laparoscopic neoadjuvant chemoradiation and preoperative adjuvant radiochemotherapy for low rectal cancer can effectively reduce serum VEGF and CEA levels and increase serum LEP levels.
