目的探讨甲状腺功能减退与妊娠糖尿病对孕妇妊娠结局的影响。方法选取2015年1月至2017年1月进行产检并正常分娩的晚期孕妇160例为研究对象,以40例妊娠糖尿病合并甲状腺功能减退者为研究组,以70例妊娠糖尿病但无甲状腺功能减退者为观察组,以50例甲状腺功能减退但无妊娠糖尿病者为对照组,3组患者均进行甲状腺功能检查,且所有患者均行口服葡萄糖耐量试验(OGTT);比较观察组与对照组患者的妊娠结局。结果研究组患者妊娠高血压发病率为20.0%,明显高于观察组与对照组的4.0%、5.7%,差异有统计学意义(P<0.05);研究组患者胎膜早破、产后出血、新生儿窒息发生率与观察组及对照组比较,研究组患者上述现象的发生率明显偏高,差异有统计学意义(P<0.05);3组间早产、胎盘早剥、新生儿低血糖发生率差异均无统计学意义(均P>0.05);观察组与对照组各妊娠结局指标比较,差异无统计学意义(P>0.05)。结论甲状腺功能减退合并妊娠糖尿病可以明显提高患者妊娠高血压的发生率,且易引发产后出血、新生儿窒息、胎膜早破等现象,临床上需加以重视,改善患者的甲状腺功能,以减少妊娠期不良妊娠结局。
Objective To investigate the effect of hypothyroidism and gestational diabetes on pregnancy outcomes in pregnant women. Methods A total of 160 pregnant women with advanced pregnancy during the period from January 2015 to January 2017 were enrolled in this study. Forty pregnant women with gestational diabetes mellitus and hypothyroidism were enrolled in this study. A total of 70 pregnant women with gestational diabetes without hypothyroidism As the observation group, 50 cases of hypothyroidism but no gestational diabetes mellitus as the control group, three groups of patients were thyroid function tests, and all patients were performed oral glucose tolerance test (OGTT); comparison between observation group and control group of patients with pregnancy ending. Results The incidence of pregnancy-induced hypertension in study group was 20.0%, which was significantly higher than 4.0% and 5.7% in observation group and control group (P <0.05). The premature rupture of membranes, postpartum hemorrhage, Compared with the observation group and control group, the incidence of neonatal asphyxia was significantly higher in the study group than in the control group (P <0.05); premature birth, placental abruption, neonatal hypoglycemia (P> 0.05). There was no significant difference in the pregnancy outcome between the observation group and the control group (P> 0.05). Conclusion Hypothyroidism combined with gestational diabetes mellitus can significantly increase the incidence of pregnancy-induced hypertension in pregnant women, and can lead to the phenomenon of postpartum hemorrhage, neonatal asphyxia and premature rupture of membranes. It is necessary to pay more attention to improve thyroid function and reduce pregnancy Bad pregnancy outcome.
