目的探讨晚期血吸虫病患者外周血Vα24自然杀伤T细胞(NKT)数量变化及其与肝纤维化程度的关系。方法采集32例晚期血吸虫病患者和23位健康体检者(对照组)静脉血各3 ml,采用流式细胞术检测外周血Vα24 NKT细胞的比例,采用酶循环法检测肝功能相关指标,同时行腹部B超检查。分析Vα24 NKT细胞比例与肝功能指标和肝纤维化程度的相关性。结果流式细胞术检测结果显示,晚期血吸虫病患者外周血Vα24 NKT细胞比例为(0.23±0.09)%,较健康对照组[(1.44±0.62)%]明显降低(P<0.01)。肝功能检测结果显示,晚期血吸虫病患者丙氨酸转氨酶(ALT)、谷酰转肽酶(γ-GT)和总胆红素(Tbil)分别为(44.78±33.42)U/L、(68.75±57.95)U/L和(20.16±11.20)μmol/L,较健康对照组[(18.77±14.19)U/L、(20.20±13.82)U/L和(11.65±5.09)μmol/L]明显增高(P<0.01,P<0.01,P<0.05);相关性分析结果显示,晚期血吸虫病患者NKT细胞比例与γ-GT呈正相关(r=0.365,P<0.05),而与ALT、天冬氨酸转氨酶(AST)、直接胆红素、碱性磷酸酶、白蛋白和白球比值等无明显相关性(P>0.05)。B超结果显示,肝纤维化Ⅰ级、Ⅱ级和Ⅲ级的晚期血吸虫病患者分别有5、11和16例,各级的Vα24 NKT细胞比例分别为(0.37±0.02)%、(0.28±0.04)%和(0.15±0.03)%,且各级间差异均有统计学意义(P<0.01);相关性分析结果显示,NKT细胞比例与肝纤维化程度呈负相关性(r=-0.91,P<0.01)。结论晚期血吸虫病患者外周血Vα24 NKT细胞数量明显降低,并随肝纤维化程度加重而减少。
Objective To investigate the changes of Vα24 NKT in peripheral blood of patients with advanced schistosomiasis and its relationship with the degree of hepatic fibrosis. Methods The venous blood samples of 32 patients with advanced schistosomiasis and 23 healthy controls (control group) were collected respectively. The proportion of Vα24 NKT cells in peripheral blood was detected by flow cytometry. The indexes of liver function were detected by enzyme-linked immunosorbent assay Abdominal B-ultrasound. The correlation between Vα24 NKT cell ratio and liver function and liver fibrosis was analyzed. Results Flow cytometry showed that the percentage of Vα24 NKT cells in peripheral blood of patients with advanced schistosomiasis was (0.23 ± 0.09)%, significantly lower than that of healthy controls (1.44 ± 0.62)%] (P <0.01). The results of liver function tests showed that ALT, γ-GT and Tbil were (44.78 ± 33.42) U / L and (68.75 ± 57.95), U / L and (20.16 ± 11.20) μmol / L were significantly higher than those in healthy controls [(18.77 ± 14.19) U / L, (20.20 ± 13.82) U / L and (11.65 ± 5.09) μmol / L The correlation analysis showed that the proportion of NKT cells in patients with advanced schistosomiasis was positively correlated with γ-GT (r = 0.365, P <0.05), but not with ALT, aspartate There was no significant correlation between AST, direct bilirubin, alkaline phosphatase, albumin and white ball ratio (P> 0.05). The results of B ultrasound showed that there were 5, 11 and 16 cases of advanced schistosomiasis grade Ⅰ, Ⅱ and Ⅲ in liver fibrosis, the percentage of Vα24 NKT cells in each grade was (0.37 ± 0.02)%, (0.28 ± 0.04) ) And (0.15 ± 0.03)%, respectively, and there were significant differences among all levels (P <0.01). Correlation analysis showed that the proportion of NKT cells was negatively correlated with the degree of liver fibrosis (r = -0.91, P <0.01). Conclusions The number of Vα24 NKT cells in peripheral blood of patients with advanced schistosomiasis is significantly decreased and decreased with the degree of liver fibrosis.
