目的:研究活血止痛软胶囊对骨折合并急性软组织损伤患者血液流变学及局部氧分压的影响,探讨其治疗骨折合并急性软组织损伤后改善局部肿胀及疼痛的作用机理。方法:收集我院2009年5月～2010年5月骨折合并急性软组织损伤住院病人112例。随机分为治疗组70例和对照组42例。治疗组常规给予口服活血止痛软胶囊,对照组给予安慰剂治疗。治疗期间停用其他治疗方法,观察治疗前后临床症状的改善情况。并于治疗前后检测各项血液流变学指标。应用中医氧分压传感针测定治疗前后局部组织氧分压的变化。结果:112例患者口服活血止痛软胶囊治疗后,患者临床症状明显改善。查体显示,软组织肿胀与疼痛均明显好转(P<0.01)。与治疗前相比,治疗组病人全血粘度及血浆粘度、胞聚集指数和刚性指数、血沉方程K值及纤维蛋白原均有所下降(P<0.05),变形指数增高(P<0.05),血球压积比较差异无统计学意义(P>0.05)。对照组患者上述指标比较差异均无统计学意义(P>0.05)。治疗前后局部氧分压治疗组差异有统计学意义(P<0.01),对照组差异无统计学意义(P>0.05)。结论:活血止痛软胶囊能通过改善局部微循环,增加局部氧供给,改善局部组织代谢达到早期活血散瘀,消肿止痛功能,疗效肯定。
Objective: To study the effect of Huoxuezhitong soft capsule on hemorheology and local oxygen partial pressure in patients with fracture and acute soft tissue injury, and to explore the mechanism of Huoxuezhitong soft capsule in treating local swelling and pain after acute soft tissue injury. Methods: A total of 112 hospitalized patients with acute soft tissue injury with fracture in our hospital from May 2009 to May 2010 were collected. Randomly divided into treatment group 70 cases and control group 42 cases. The treatment group was given routine oral huoxuezhitong soft capsules, the control group was given placebo treatment. Disable other treatment during treatment to observe the improvement of clinical symptoms before and after treatment. And before and after treatment to detect the various indicators of hemorheology. Application of TCM partial pressure of oxygen sensor to measure changes of local tissue oxygen partial pressure before and after treatment. Results: The clinical symptoms of 112 patients after oral administration of Huoxue Zhitong soft capsule were significantly improved. Examination showed that soft tissue swelling and pain were significantly improved (P <0.01). Compared with before treatment, whole blood viscosity, plasma viscosity, cell aggregation index and rigidity index, erythrocyte sedimentation index K value and fibrinogen in treatment group decreased (P <0.05) and deformation index increased (P <0.05) There was no significant difference in hematocrit (P> 0.05). There was no significant difference between the above indexes in the control group (P> 0.05). Before and after treatment of partial oxygen partial pressure treatment group differences were statistically significant (P <0.01), the control group no significant difference (P> 0.05). Conclusion: Huoxue Zhitong soft capsule can improve local microcirculation, increase local oxygen supply, improve local tissue metabolism to achieve early blood stasis, swelling and analgesic function, and the curative effect is certain.
